Repace H (12.5mg+50mg) Tablet is indicated for the treatment of hypertension.  This medication contains Losartan and Hydrochlorothiazide as working ingredients. Additionally, it is prescribed to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.

✔️ Pharmacology
  • Angiotensin II: A potent vasoconstrictor, a key vasoactive hormone in the renin-angiotensin system, and a crucial element in hypertension pathophysiology.
  • Losartan: Blocks the binding of angiotensin II to the AT1 receptor, reducing its vasoconstrictor and aldosterone-secreting effects.
  • Hydrochlorothiazide: A thiazide diuretic impacting electrolyte reabsorption in kidney tubules, leading to increased sodium and chloride excretion.
✔️ Dosage of Hypertension:
  • Initial dose: 50/12.5, one tablet once daily.
  • Inadequate response: Increase to 100/25 once daily.
  • Inadequate control with Losartan 100 mg monotherapy: Switch to 100/12.5 once daily.
  • Hypertensive patients with left ventricular hypertrophy: Initial dose 50/12.5, may increase to 100/12.5 and then to 100/25 if needed. Max dose: 100/25 once daily.
  • Antihypertensive effects are usually achieved within three weeks.
  • No initial dosage adjustment for elderly patients. Max dose of 100/25 not for initial therapy in the elderly.
✔️ Dosage of Severe Hypertension:
  • Initial treatment: 50/12.5 once daily.
  • Inadequate response after 2 to 4 weeks: Increase to 100/25 once daily. Max dose: 100/25 once daily.
✔️ Administration of

May be administered with other antihypertensive agents. Can be taken with or without food.

✔️ Interaction:
  • Losartan Potassium: No significant pharmacokinetic interactions were found with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, and Phenobarbital.
  • Concomitant use with potassium-sparing diuretics, potassium supplements, or salt replacements containing potassium may increase serum potassium.
  • Interaction with Indomethacin may decrease antihypertensive effects.

  • Contraindicated in patients hypersensitive to either component.
  • Contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived pharmaceuticals (Hydrochlorothiazide).

  • Not recommended during pregnancy.
  • Not recommended for nursing mothers.

  • Hypersensitivity reactions, including angioedema.
