✅ Description:

Linagliptin + Metformin Hydrochloride is a combination medication containing Linagliptin (a DPP-4 inhibitor) and Metformin (a Biguanide), belonging to the anti-diabetic class. In type 2 diabetes, the body either doesn't produce enough insulin or becomes resistant to its effects. This medication addresses these issues.

Linagliptin, a DPP-4 inhibitor, blocks an enzyme that breaks down incretin hormones, which regulate insulin production. It also reduces unnecessary sugar production by the liver. Metformin, on the other hand, lowers glucose production in the liver, slows sugar absorption from the intestines, and enhances insulin sensitivity. Together, they prevent excessive blood sugar levels after meals.

Take Linagliptin + Metformin Hydrochloride with food to avoid stomach upset. Consistency in timing is important. Dosage adjustments should be based on your doctor's guidance, as they may change according to your condition. Common side effects include hypoglycemia (low blood sugar) symptoms like dizziness and sweating. To prevent this, avoid skipping meals and keep sugar handy. Other side effects include taste changes, nausea, diarrhea, stomach pain, headache, and respiratory symptoms.

Do not discontinue Linagliptin + Metformin Hydrochloride abruptly, even if you feel better, as blood sugar levels fluctuate. Stopping suddenly can raise the risk of complications like eyesight loss, kidney problems, and nerve damage. This medication is not suitable for type 1 diabetes or severe kidney/liver disease. Inform your doctor about heart disease, pregnancy plans, or breastfeeding.

✔️ How does Adlinameg 2.5/500mg Tablet work?

Adlinameg 2.5/500mg Tablet operates through the synergy of its active components. Metformin reduces liver glucose production, delays sugar absorption in the intestine, and enhances insulin responsiveness. Linagliptin boosts insulin secretion from the pancreas and curbs hormones elevating blood sugar. As a result, both fasting and post-meal blood sugar levels are lowered, contributing to effective blood sugar control.

✔️ Quick Suggestions:
  • Metformin + Linagliptin should be taken with food to lower the chances of having an upset stomach.
  • Take it in addition to regular exercise and a healthy diet to lower the levels of blood sugar.
  • Chances of weight gain and low blood sugar are lesser as compared to other diabetes medicines.
  • Hypoglycemia (low blood sugar level) may occur when taken along with other antidiabetic medicines, alcohol, or by delaying/skipping a meal. Carry a sugar source with you for immediate relief.
  • Monitor your blood sugar level regularly while you are taking this medicine.
  • Inform your doctor about your diabetes treatment if you are due to have any surgery under a general anesthetic.
  • Tell your doctor immediately if you experience any deep or rapid breathing, persistent nausea, vomiting, or stomach pain as Metformin + Linagliptin may cause a rare but serious condition called lactic acidosis, which is an excess of lactic acid in the blood.
✔️ Indication

Type 2 Diabetes Mellitus (DM) arises from elevated blood sugar levels due to reduced insulin production and inadequate response to insulin in your body. Adlinameg 2.5/500mg Tablet is employed to manage Type 2 Diabetes Mellitus by addressing these issues.

✔️ Pharmacology

Linagliptin is prescribed to assist individuals with type 2 diabetes in regulating their blood sugar levels. It inhibits DPP-4 (dipeptidyl peptidase-4), an enzyme responsible for breaking down GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide), which are incretin hormones. By inhibiting DPP-4, Linagliptin enhances the release of insulin from pancreatic beta cells in response to glucose levels. Additionally, it reduces the production of glucagon from pancreatic alpha cells, leading to higher active incretin hormone concentrations.

Metformin Hydrochloride is a medication from the biguanide class used to treat type 2 diabetes. It affects both baseline and postprandial (after-meal) plasma glucose levels. Unlike sulfonylureas, it doesn't cause hypoglycemia. Metformin Hydrochloride acts by lowering hepatic glucose synthesis, decreasing intestinal glucose absorption, and enhancing insulin sensitivity. This is achieved by boosting peripheral glucose uptake and utilization.

✔️ Dosage & Administration of Adlinameg 2.5/500mg Tablet
  • The dosing of Adlinameg, an antihyperglycemic therapy combining linagliptin and metformin hydrochloride, should be personalized based on the patient's existing regimen, effectiveness, and tolerability. The maximum recommended daily dose is 5 mg of linagliptin and 2000 mg of metformin hydrochloride.
  • For patients insufficiently controlled on metformin monotherapy: The initial dose of Adlinameg should include linagliptin at 2.5 mg twice daily (5 mg total daily dose) in addition to the ongoing metformin dose.
  • For patients switching from separate linagliptin and metformin therapy: Adlinameg should begin with the same dose of linagliptin and metformin that the patient was previously taking.
  • For patients inadequately controlled on dual therapy of metformin and sulphonylurea: Adlinameg should comprise linagliptin at 2.5 mg twice daily (5 mg total daily dose) and a metformin dose comparable to the ongoing one. In such cases, the sulphonylurea dose might need adjustment to lower the risk of hypoglycemia.
  • For patients inadequately controlled on dual therapy of insulin and metformin: Adlinameg should include linagliptin at 2.5 mg twice daily (5 mg total daily dose) and a metformin dose similar to the ongoing one. Adjusting the insulin dose may be necessary to decrease the risk of hypoglycemia.
  • Elderly patients: Caution should be exercised as age increases due to metformin being excreted by the kidneys. Renal function monitoring is crucial, particularly for patients over 80 years old.
  • Renal impairment: Adlinameg should not be used in patients with moderate or severe renal impairment (creatinine clearance < 60 ml/min) due to metformin.
  • Hepatic impairment: Adlinameg is not recommended for patients with hepatic impairment due to metformin. Limited clinical experience exists for hepatically impaired patients.
  • Pediatric population: The safety and efficacy of Adlinameg have not been established for children and adolescents aged 0 to 18 years.
  • Adlinameg should be taken twice daily with meals to reduce gastrointestinal adverse reactions linked to metformin. Patients should maintain their carbohydrate intake throughout the day. Overweight patients should adhere to their energy-restricted diet.
  • If a dose is missed, take it as soon as remembered. Avoid taking a double dose at once; if missed, skip the dose.

✔️ Precautions & Warnings
  • Linagliptin + Metformin Hydrochloride is contraindicated in patients with type 1 diabetes or diabetic ketoacidosis. Some diabetic patients taking LINAGLIPTIN+METFORMIN might experience a rare but serious condition called lactic acidosis, where excess lactic acid accumulates in the blood, potentially damaging the liver and kidneys responsible for its elimination.
  • Before starting Linagliptin + Metformin Hydrochloride, inform your doctor if you have a history of pancreatitis, kidney disease, or low vitamin B12 levels. When used alongside insulin, Linagliptin + Metformin Hydrochloride may significantly lower blood sugar levels, leading to potentially fatal hypoglycemia. In such cases, your doctor may adjust the insulin or Linagliptin + Metformin Hydrochloride dosage.
  • Prolonged use of Linagliptin + Metformin Hydrochloride may lower thyroid-stimulating hormone (TSH), making annual TSH checks advisable. Rarely, a serious skin reaction called bullous pemphigoid may occur, necessitating immediate medical attention.
  • If you are scheduled for a diagnostic test involving dye or X-ray contrast agents, inform your doctor. The use of Linagliptin + Metformin Hydrochloride should be temporarily stopped before undergoing an X-ray procedure.
