Pharmacology

Ertugliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor that reduces glucose reabsorption in the kidneys, leading to increased urinary glucose excretion.

Dosage & Administration of Eltero 5
  • Adults:
    • Starting dose: 5 mg once daily, taken in the morning (with or without food).
    • Maximum dose: 15 mg once daily if additional control is needed.
  • Renal Impairment:
    • Contraindicated in eGFR < 30 mL/min/1.73 m².
    • Not recommended to initiate in eGFR 30-60 mL/min/1.73 m².
  • Hepatic Impairment: No dose adjustment for mild or moderate impairment; not recommended for severe impairment.
  • Pediatrics: Safety and efficacy not established in those under 18 years of age.

Pregnancy:

    • Doses approximately 13 times the maximum clinical dose led to renal pelvic and tubule dilatations and renal mineralization that were not fully reversible.
    • No evidence of fetal harm was observed in rats or rabbits at exposures around 300 times the maximum clinical dose (15 mg/day) during organogenesis.
  • Human Data: Limited data are available on pregnant women, and they are insufficient to determine a drug-associated risk of adverse developmental outcomes. However, there are inherent risks to the mother and fetus associated with poorly controlled diabetes during pregnancy.

Lactation:

  • Recommendation: Not recommended while breastfeeding.
  • Distribution in Breast Milk: It is unknown if ertugliflozin is distributed in human breast milk.
  • Concerns: Human kidney maturation occurs in utero and during the first two years of life. There may be risks to the developing human kidney due to potential exposure during lactation.
  • Advice for Breastfeeding Women: Due to the potential for serious adverse reactions in a breastfed infant, it is advised that ertugliflozin is not recommended during breastfeeding.
Precautions & Warnings
  • Intravascular Volume Contraction: This medication may lead to a decrease in blood volume; symptomatic hypotension may occur, especially in individuals with renal impairment, low systolic blood pressure, those on diuretics, or elderly patients.
  • Ketoacidosis Risk: Ketoacidosis, a serious and life-threatening condition, has been reported in patients taking SGLT2 inhibitors. Before starting treatment, assess any factors that might increase the risk for ketoacidosis, such as pancreatic insulin deficiency, caloric restriction, or alcohol use. Monitor for signs of ketoacidosis and consider temporary discontinuation in situations that could predispose a patient to this condition, such as prolonged fasting due to illness or surgery.
  • Renal Impairment Risk: There is a potential for acute kidney injury due to intravascular volume contraction. Before starting, consider any predisposing factors such as hypovolemia, chronic renal insufficiency, congestive heart failure, or the use of certain medications (e.g., diuretics, ACE inhibitors, ARBs, NSAIDs). It may be necessary to temporarily stop Ertugliflozin if there is reduced fluid intake or fluid loss and to monitor for signs of acute kidney injury.
  • Genital Mycotic Infections: There may be an increased occurrence of genital fungal infections, particularly in patients with a history of such infections or uncircumcised males.
  • Urinary Tract Infections: Serious urinary tract infections, including urosepsis and pyelonephritis requiring hospitalization, have been reported in patients taking SGLT2 inhibitors. There is an increased risk for urinary tract infections (UTIs), which can lead to severe complications like urosepsis and pyelonephritis starting from UTIs.
