• It may also reduce the risk of heart attack, heart failure, or other cardiovascular events.
• You should continue to exercise regularly, eat a healthy diet, and take your other diabetes medicines (if prescribed) along with Empagliflozin + Linagliptin.
• Take it with food to lower your chance of having an upset stomach.
• Practice good genital hygiene to prevent yeast or urinary tract infections
• It may cause your body to lose too much fluid (dehydration) or you urinate more often. Drink plenty of water, stay hydrated, and rise slowly if you have been sitting or lying down.
• It can cause hypoglycemia (low blood sugar level) when used with other antidiabetic medicines, alcohol, or if you delay or miss a meal. Check your blood sugar regularly.
• Always carry some sugary food or fruit juice with you in case you experience hypoglycemia symptoms such as cold sweats, cool pale skin, tremors, and anxious feelings.
• Stop taking Empagliflozin + Linagliptin and Inform your doctor if you notice joint pains or develop symptoms such as stomach pain, nausea, and/or vomiting.

Indication

It is used to manage type II diabetes mellitus among adult patients when diet, exercise and management with other conventional medicines does not result in adequate sugar control. It is used as an adjunct therapy along with proper diet and exercise to achieve better results.         

Pharmacology

• Use: Empagliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor used to treat high blood pressure and diabetes. It works by inhibiting SGLT2, reducing the reabsorption of glucose in the kidneys, and increasing glucose excretion in the urine.

Linagliptin:

• Use: Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that helps manage diabetes. It increases levels of incretin hormones, which stimulate insulin release and decrease glucagon levels, aiding in glucose regulation.

Dosage & Administration:

• Adults: 5 mg once daily. It can be taken with or without meals at any time of the day.
• With Metformin: Maintain the metformin dose and take linagliptin together.
• With Sulphonylurea/Insulin: Consider reducing the dose of sulphonylurea or insulin to lower hypoglycemia risk.
• Missed Dose: Take as soon as remembered, but do not double the dose on the same day.
• Renal Impairment: No dose adjustment is needed.
• Hepatic Impairment: No dose adjustment is needed, but clinical experience is limited.
• Elderly: No adjustment is needed based on age, but use caution in patients over 80 years old.
• Pediatric: Safety and efficacy not established.

Interaction

• Metformin: No significant interaction.
• Sulphonylureas: No change in pharmacokinetics with glibenclamide.
• Other Medications: No clinically relevant effects on metformin, glyburide, simvastatin, warfarin, digoxin, or oral contraceptives.

Contraindications

Hypersensitivity: To active substances or any excipients.

Pregnancy & Lactation

Avoid use during pregnancy and breastfeeding unless necessary. Weigh the benefits of breastfeeding against the need for therapy.

Precautions & Warnings

• Pancreatitis: Discontinue if suspected.
• Hypotension: Assess and correct volume status before starting therapy, especially in patients with renal impairment or low blood pressure.
• Renal Function: Monitor during therapy, especially with eGFR below 60 mL/min/1.73 m².
• Hypoglycemia: Consider lowering insulin doses when starting this medication.
• Genital Mycotic Infections & UTIs: Monitor and treat as needed.
• Hypersensitivity: Discontinue and assess for other causes, and initiate alternative treatment.
• Increased LDL-C: Monitor and treat as needed.
• Macrovascular Outcomes: No conclusive evidence of risk reduction.
