How does Glims 2 work?

Glimepiride works by increasing the amount of insulin released by the pancreas to lower blood glucose.

If you experience symptoms of low blood sugar such as sweating, dizziness, shaking, or headaches, it is important to address it immediately by consuming a fast-acting source of glucose like candy. Missing meals can also lead to hypoglycemia, so regular meals are crucial while on Glimepiride.

Quick Suggestions:

• Exercise regularly, eat a healthy diet, and take your other diabetes medicines (if prescribed) alongside.
• It can cause hypoglycemia (low blood sugar level) when used with other antidiabetic medicines, alcohol, or if you delay or miss a meal.
• Always carry some sugary food or fruit juice with you in case you experience hypoglycemic symptoms such as cold sweats, cool pale skin, tremorsand anxiety.
• Your doctor may check your liver function regularly. Inform your doctor if you develop symptoms, such as abdominal pain, loss of appetite, or yellowing of the eyes or skin (jaundice).
• Monitor your blood sugar level regularly while you are taking Glimepiride.

Glims is prescribed for the following conditions:

• It is used as an additional treatment to diet and exercise to lower blood sugar in patients with Type II diabetes mellitus (non-insulin-dependent), where diet and exercise alone are insufficient to control hyperglycemia.
• Glims can be used with metformin when diet, exercise, and either drug alone do not provide adequate blood sugar control.
• It is also indicated for use in combination with insulin in patients whose blood sugar cannot be managed with diet, exercise, and oral hypoglycemic agents. However, combining Glims with insulin increases the risk of hypoglycemia.

Pharmacology:

Glimepiride, a sulfonylurea antidiabetic agent, lowers blood sugar by stimulating insulin release from functional pancreatic beta cells. It improves beta cells' sensitivity to glucose, enhancing insulin secretion. Additionally, Glimepiride reduces hepatic glucose production, improves peripheral tissue insulin sensitivity, and increases glucose uptake. Its effects can last up to 24 hours in non-fasting diabetic patients.

Dosage & Administration of Glims 2

The dosage of Glimepiride depends on the desired blood sugar level, and the lowest effective dose should be used to maintain metabolic control. Regular blood and urine glucose checks are needed for dose adjustments.

• Initial dose and titration: Typically, the starting dose is 1 mg daily, with gradual increases based on blood sugar monitoring, adjusting the dose every 1 to 2 weeks: 1 mg -> 2 mg -> 3 mg -> 4 mg -> 6 mg.
• Maintenance dose for controlled diabetes: The usual dose range is 1 to 4 mg per day.
• Timing of doses: Normally, one dose is taken daily, immediately before a substantial breakfast or the first main meal. Skipping meals after taking Glimepiride is not advised.
• Secondary dose adjustments: As diabetes improves, insulin sensitivity increases, so Glimepiride dosage may need to be lowered. Dose adjustments may also be necessary with changes in weight, lifestyle, or other factors affecting blood sugar.
• Switching from other antidiabetic drugs: When switching to Glimepiride from other oral antidiabetics, the initial dose is 1 mg daily, regardless of the previous drug's dose. Dose increases should follow the same titration guidelines as for new patients.

Glimepiride tablets should be taken with enough water.

Interaction

Certain drugs may interact with Glimepiride, either enhancing or reducing its hypoglycemic effect:

• Drugs that enhance hypoglycemic effect: Include insulin, oral antidiabetic agents, ACE inhibitors, anabolic steroids, and some antibiotics (e.g., chloramphenicol).
• Drugs that reduce hypoglycemic effects: Include corticosteroids, diuretics, glucagon, and thyroid hormones.

Certain drugs, like beta-blockers, clonidine, and alcohol, can mask the warning signs of hypoglycemia or unpredictably affect Glimepiride’s activity.

Contraindications

Glimepiride is not suitable for treating insulin-dependent (Type I) diabetes, diabetic ketoacidosis, or diabetic coma. It is contraindicated in patients with severe liver or kidney dysfunction, those undergoing dialysis, or those allergic to Glimepiride, sulfonylureas, or sulfonamides.

Pregnancy & Lactation

Glimepiride should not be taken during pregnancy, and insulin should be used instead. If breastfeeding, Glimepiride should be discontinued, as it can harm the child.

Precautions & Warnings

During the initial weeks of treatment, the risk of hypoglycemia is higher, so careful monitoring is required. Immediate intake of glucose or sugar can help control hypoglycemia.
