Liglimet 2.5/850mg Tablet is used alongside diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus when treatment with both Linagliptin and Metformin Hydrochloride is suitable.

Pharmacology
  • Linagliptin: Used to improve glycemic control in type 2 diabetes patients by inhibiting DPP-4 (dipeptidyl peptidase-4), which degrades incretin hormones (GLP-1 and GIP). This results in increased levels of active incretin hormones, promoting insulin release from pancreatic beta cells and reducing glucagon secretion from alpha cells.
  • Metformin Hydrochloride: An oral antihyperglycemic drug (biguanide class) used for managing type 2 diabetes by lowering both basal and postprandial plasma glucose. Unlike sulfonylureas, it doesn’t cause hypoglycemia. Metformin decreases liver glucose production, reduces intestinal glucose absorption, and enhances insulin sensitivity by increasing peripheral glucose uptake and utilization.
Dosage & Administration of Liglimet 2.5/850mg Tablet

Immediate Release Tablets:

  • The dosage should be personalized based on effectiveness and tolerability.
  • Maximum recommended dose: 2.5 mg of Linagliptin and 1000 mg of Metformin Hydrochloride twice daily with meals.

  • Patients already on separate Linagliptin and Metformin can switch to this combination with equivalent doses.

  • Dosage should be personalized, not exceeding 5 mg of Linagliptin and 2000 mg of Metformin Hydrochloride daily.
  • Start with 5 mg Linagliptin/1000 mg Metformin Hydrochloride extended-release once daily with a meal for those not previously on Metformin.
  • For those already on Metformin, start with 5 mg Linagliptin daily and an equivalent Metformin dose once daily.
  • Patients on immediate-release tablets can switch to extended-release tablets containing 5 mg Linagliptin and an equivalent Metformin dose once daily.
  • Extended-release tablets of 5 mg Linagliptin/1000 mg Metformin should be taken as a single tablet once daily, or two 2.5 mg Linagliptin/1000 mg Metformin tablets together once daily.
Interaction
  • Cationic Drugs: Drugs such as amiloride, digoxin, morphine, and others that are excreted via renal tubular secretion might interact with Metformin. Monitor patients carefully and adjust doses if necessary.
  • Carbonic Anhydrase Inhibitors: Drugs like topiramate may induce metabolic acidosis, increasing the risk of lactic acidosis. Use with caution.
