Linagliptin helps to control blood sugar levels by increasing substances in the body that make the pancreas release more insulin. Metformin reduces the absorption of sugar from the stomach, reduces the release of stored sugar from the liver, and helps your body use sugar better.

• Common: Nasopharyngitis and diarrhea.
• Hypoglycemia is more common when used with sulfonylureas.

Pharmacology

Linagliptin:

• Lowers both basal and postprandial plasma glucose.

Dosage & Administration of Lijenta-M 500mg Tablet

Immediate Release Tablets:

• Dosage is individualized based on effectiveness and tolerability.
• Max recommended dose: 2.5 mg of Linagliptin and 1000 mg of Metformin twice daily with meals.
• Starting dose for new patients: 2.5 mg Linagliptin and 500 mg Metformin twice daily.
• Existing Metformin users: 2.5 mg Linagliptin with current Metformin dose twice daily.
• Switching from individual components to combination: Match doses of each component.

Extended-Release Tablets:

• Dosage is individualized based on effectiveness and tolerability.
• Max recommended total daily dose: 5 mg Linagliptin and 2000 mg Metformin.
• Starting dose for new patients: 5 mg Linagliptin/1000 mg Metformin once daily with a meal.
• Existing Metformin users: 5 mg Linagliptin with a similar Metformin dose once daily with a meal.
• Switching from the immediate release: Match the total daily dose of each component to the extended-release form.

Interaction

• Cationic Drugs: Potential interaction with Metformin via renal tubular secretion; monitor patients closely.
• Carbonic Anhydrase Inhibitors: Risk of metabolic acidosis; use cautiously with Metformin.
• Inducers of P-glycoprotein and CYP3A4 Enzymes: May reduce Linagliptin efficacy; consider alternative treatments.

Contraindications

Not suitable for patients with renal impairment, acute or chronic metabolic acidosis, or hypersensitivity to Linagliptin or Metformin.
