Lino-M (2.5mg+500mg) Tablet is a combination of two antidiabetic medicines: Metformin and Linagliptin. 

• Metformin is an antidiabetic medication (biguanide). It works by lowering the glucose production in the liver, delaying glucose absorption from the intestines, and increasing the body's sensitivity to insulin.
• Linagliptin is a DPP-4 inhibitor that works by increasing the release of insulin from the pancreas and decreasing the hormones that raise blood sugar levels. This reduces both fasting and postmeal sugar levels. Together, they provide better control of blood sugar.

• Common: Nasopharyngitis, diarrhea, hypoglycemia (more common with sulfonylureas).
• Serious: Lactic acidosis, pancreatitis.

Pharmacology

• Linagliptin: Inhibits DPP-4 (dipeptidyl peptidase-4), preventing the degradation of incretin hormones (GLP-1 and GIP). This leads to increased insulin release and decreased glucagon secretion, improving glycemic control.
• Metformin Hydrochloride: A biguanide that lowers basal and postprandial plasma glucose by decreasing hepatic glucose production, reducing intestinal absorption of glucose, and improving insulin sensitivity.

Dosage & Administration of Lino-M (2.5mg+500mg) Tablet

Immediate Release Tablets:

• Maximum Dose: 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals.
• Starting Dose:
  • For patients not currently treated with Metformin: 2.5 mg Linagliptin and 500 mg Metformin twice daily.
  • For patients currently treated with Metformin: 2.5 mg Linagliptin and the current Metformin dose twice daily.

Extended-Release Tablets:

• Maximum Dose: 5 mg Linagliptin and 2000 mg Metformin Hydrochloride once daily with a meal.
• Starting Dose:
  • For patients not currently treated with Metformin: 5 mg Linagliptin/1000 mg Metformin once daily.
  • For patients currently treated with Metformin: 5 mg Linagliptin and the current Metformin dose once daily.
  • For patients switching from immediate release: Match total daily doses.

Overdose Effects

• Supportive Measures: Remove unabsorbed material, monitor clinically, and provide supportive treatment.
• Hemodialysis: Unlikely to remove Linagliptin but effective for Metformin.

Interaction

• Cationic Drugs: Amiloride, digoxin, morphine, etc., may compete for renal tubular secretion, requiring monitoring and possible dose adjustments.
• Carbonic Anhydrase Inhibitors: Drugs like topiramate may induce metabolic acidosis, increasing lactic acidosis risk.
• Inducers of P-glycoprotein and CYP3A4 Enzymes: Rifampin can reduce Linagliptin exposure; alternative treatments are recommended.
• Drugs Affecting Glycemic Control: Thiazides, corticosteroids, thyroid products, etc., may induce hyperglycemia; monitor closely.

Contraindications

• Renal Impairment: Risk of Metformin accumulation and lactic acidosis.
• Acute/Chronic Metabolic Acidosis: Including diabetic ketoacidosis.
• Hypersensitivity: To Linagliptin or Metformin Hydrochloride.

Pregnancy & Lactation

• Pregnancy: Use only if clearly needed due to lack of adequate studies.
• Lactation: Exercise caution; effects are not well studied.

Precautions & Warnings

• Lactic Acidosis: Discontinue immediately if suspected.
• Pancreatitis: Discontinue if suspected.
• Temporary Discontinuation: Necessary before radiologic studies with contrast materials or major surgery.
• Vitamin B12 Monitoring: Annual hematologic parameter checks.
