Indication of Metform 850mg Tablet

Type 2 Diabetes Mellitus:

• Primary Use: Treatment, especially in overweight patients when diet and exercise alone are insufficient for glycaemic control.
• Adults: Can be used as monotherapy or with other oral antidiabetic agents or insulin.
• Children (10+ years) and Adolescents: Can be used as monotherapy or with insulin.
• Reduction in Complications: Shown to reduce diabetic complications in overweight adults when used as first-line therapy after diet failure.

Pharmacology

Dosage & Administration of Metform 850mg Tablet

Immediate Release Tablet:

• Adults:
  • Starting dose: 500 mg twice daily or 850 mg once daily with meals.
  • Increase: 500 mg weekly or 850 mg every 2 weeks, up to 2000 mg daily in divided doses.
  • Maximum: 2550 mg daily for additional glycemic control.
  • Doses above 2000 mg should be split into three meals.
• Children:
  • Starting dose: 500 mg twice daily with meals.
  • Increase: 500 mg weekly up to 2000 mg daily in divided doses.

Extended Release Tablet:

• Adults:
  • Starting dose: 500 mg once daily with the evening meal.
  • Increase: 500 mg weekly up to 2000 mg once daily or 1000 mg twice daily with meals.
  • Transition: Immediate release to extended-release should not exceed the recommended maximum daily dose.
• Children: Not studied for extended-release.
• Elderly: Adjust dosage based on renal function.
• Children: Diagnosis confirmation is needed before treatment, careful follow-up is recommended.
• Renal Function: Regular monitoring; contraindicated if GFR < 30 mL/min.

Overdose Effects

• Conditions Causing Tissue Hypoxia: Cardiac or respiratory failure, recent myocardial infarction, hepatic insufficiency, acute alcohol intoxication, alcoholism.

Pregnancy & Lactation

• Pregnancy: Insulin is preferred over metformin to maintain near-normal blood glucose.
• Breastfeeding: Metformin is excreted in breast milk; breastfeeding  is not recommended during treatment.
