How does Amarin 22.7mg Tablet work?

Pheniramine competes with histamine for H1 receptors, reducing itching, vasodilation, and capillary leakage. It also has sedative effects due to its action on the CNS and may reduce inflammation by antagonizing H4 receptors.

Indication

Pheniramine Maleate belongs to the class of medications called 'anti-histamine' primarily used to treat allergic conditions, such as hayfever, runny nose, allergic conjunctivitis, rashes, serum sickness, food allergy, vasomotor rhinitis, acute rhinitis, itching skin conditions, such as neurodermatitis, eczema, lichen planus, urticaria, pruritis, and radiation sickness. Additionally, Pheniramine Maleate is also used to prevent and treat motion sickness, nausea, vomiting, and vertigo due to Menière's disease and other labyrinthine disturbances. Pheniramine Maleate contains Pheniramine maleate, which works by blocking the action of a chemical messenger called 'histamine', which is responsible for causing allergic reactions. Thereby Pheniramine Maleate treats allergic conditions.

Pharmacology

Pheniramine works by competing with histamine for the H1 receptor and acts as an inverse agonist once it binds. This action leads to a decrease in H1 receptor activity, which in turn reduces itching, vasodilation, and capillary leakage, ultimately leading to less redness and swelling. This effect is particularly evident in the suppression of the histamine-induced wheal (swelling) and flare (vasodilation) response.

In the central nervous system (CNS), the inverse agonism of the H1 receptor by pheniramine is responsible for the sedative effects commonly associated with first-generation antihistamines. Additionally, pheniramine's binding to H4 receptors and its inverse agonism at these sites may also help reduce itching by counteracting inflammation.

Dosage & Administration of Amarin 22.7mg Tablet

• Adults and Adolescents (12 years and over): 1.0-2.0 ml IV/IM, administered once or twice daily.
• Children (1-11 years): Doses vary based on age and weight, administered intramuscularly only. For example, children aged 1-2 years may receive 0.5-0.7 ml, while those aged 6-11 years may receive 1.0-1.5 ml.

Tablet (22.7 mg):

• Adults and Adolescents (12 years and over): Start with 1 tablet 2-3 times daily after meals, which may be increased to 2 tablets 3 times daily if necessary.

Syrup (15 mg/5 ml):

• Children (1-3 years): Half a measuring spoonful 2-3 times daily.
• Children (4-12 years): 1 measuring spoonful 2-3 times daily.
• Adults and Adolescents: 1-2 measuring spoonfuls 2-3 times daily after meals.

Retard Tablet (75 mg):

• Adults and Adolescents (12 years and over): 1 coated tablet after supper, with prolonged action lasting up to 24 hours. In resistant cases, an additional tablet may be taken in the morning.

Interaction

• MAO Inhibitors: May prolong and intensify the anticholinergic effect of pheniramine.
• CNS Depressants: Alcohol and other CNS depressants like hypnotics, sedatives, and tranquilizers may enhance the adverse CNS effects of pheniramine.
• Atropine: This may enhance the anticholinergic activity of pheniramine.

Contraindications

• Patients with hypersensitivity to pheniramine or any ingredient in the formulation, symptomatic prostatic hypertrophy, those on MAO inhibitor therapy, and newborns or premature infants should not use Amarin 22.7 mg.

Pregnancy & Lactation

• Pregnancy Category C: Use only if the potential benefit justifies the potential risk to the fetus. This classification indicates that animal studies have shown adverse effects on the fetus, and there are no well-controlled studies in humans.

Precautions & Warnings

Amarin may cause drowsiness, so patients should be cautious when driving or operating machinery. Patients should avoid alcohol and other CNS depressants while on this medication. Amarin has the potential for abuse and should be used with caution in patients with prostatic hypertrophy, narrow-angle glaucoma, asthma, or severe cardiovascular disease. The anti-emetic effect may mask symptoms of other conditions.

Storage Conditions:

Store in a cool, dry place, protected from light. Keep out of the reach of children and do not use beyond the expiry date. Amarin should only be dispensed on the prescription of a registered physician.
