Rupatadine acts as a non-sedative antagonist at histamine H1-receptors, and it has an extended half-life. Additionally, it inhibits platelet-activating factor (PAF). Both histamine and PAF contribute to bronchoconstriction, and increased vascular permeability, and act as mediators in the inflammatory process. Rupatadine's dual mechanism of action provides a more effective therapeutic impact compared to a single antihistamine.
Moreover, Rupatadine inhibits mast cell degranulation triggered by both immunological and non-immune stimuli. It also suppresses the release of cytokines, particularly tumor necrosis factor-alpha (TNF), in human mastocytes and monocytes. These actions contribute to its efficacy in treating allergic conditions and inflammatory responses.
When administered concurrently with ketoconazole or erythromycin, the systemic exposure of Rupatadine increases. Caution is advised when using Rupatadine with these medications and other CYP3A4 isozyme inhibitors. Additionally, caution is recommended when co-administering Rupatadine with statins, CNS depressants, or alcohol. Avoid consuming grapefruit or grapefruit juice simultaneously with Rupatadine.
