Desloratadine may cause fever, irritability, headache, diarrhea, cough, upper respiratory tract infection, pharyngitis, dry mouth, myalgia, emotional lability, erythema, maculopapular rash, dizziness, fatigue, somnolence, urinary tract infection, dyspepsia, insomnia, and dysmenorrhea. Pseudoephedrine side effects may include CNS effects, insomnia, arrhythmia, hypotension, tachycardia, rash, urticaria, anorexia, xerostomia, dysuria, polyuria, nausea, vomiting, and ischemic colitis.
Desloratadine is a long-acting, non-sedating, selective peripheral histamine H1-receptor antagonist. Pseudoephedrine acts as both an α- and β-adrenergic receptor agonist, causing vasoconstriction and bronchial relaxation.
Potential increase in plasma concentrations with drugs affecting hepatic microsomal enzymes (e.g., azithromycin, cimetidine, erythromycin, fluoxetine, ketoconazole). Increased risk of hypertension and arrhythmias with cardiac glycosides, quinidine, or TCAs. Increased risk of vasoconstrictor effects with ergot alkaloids or oxytocin. Co-administration with MAOIs may cause hypertensive crisis. Interaction with anesthetics and antihypertensive agents.
Hypersensitivity.
Desloratadine: Limited data in pregnant women; use only if needed. Pseudoephedrine: The majority of studies found no association with an increased risk of congenital anomalies. Use during lactation may decrease milk production and cause irritability in infants.
Severe renal or hepatic failure; children under 6 months, elderly, epilepsy, pregnancy, and lactation. Pseudoephedrine concentrates in breast milk; use caution during lactation.
Store in a cool, dry place, protected from light.
