How does Embardia 60mcg/0.3ml Injection work?

N increases red blood cell counts by stimulating the bone marrow (soft tissue inside the bones where blood is made) to make more red blood cells.

Indication of Embardia 60mcg/0.3ml Injection

Embardia 60mcg/0.3ml Injection is a human-made form of naturally occurring erythropoiesis-stimulating protein or erythropoietin (a hormone produced by the kidney's specialized cells and is responsible for protecting red blood cells against destruction and also stimulates the cells of bone marrow to produce more amount of red blood cells). 

Pharmacology

Darbepoetin alfa is an erythropoiesis-stimulating agent (ESA). It is produced through recombinant DNA technology in Chinese Hamster Ovary (CHO) cells. It stimulates the production of red blood cells by acting on erythroid progenitor cells, similar to erythropoietin. Darbepoetin alfa differs from recombinant human erythropoietin by having five N-linked oligosaccharide chains (compared to three), leading to an extended half-life and prolonged activity.

Dosage of Embardia 60mcg/0.3ml Injection

Chronic Renal Failure (Adult Patients):

• Correction phase: Initial dose of 0.45 µg/kg once weekly via subcutaneous or intravenous route. For patients not on dialysis, 0.75 µg/kg can be given every two weeks or 1.5 µg/kg once monthly.
• Maintenance phase: This can be administered weekly, biweekly, or once monthly depending on the patient's hemoglobin level and dialysis status.

Chronic Renal Failure (Pediatric Patients ≥1 year):

• Correction phase: 0.45 µg/kg once weekly subcutaneously or intravenously, or 0.75 µg/kg every two weeks for non-dialysis patients.
• Maintenance phase: Same as adults, with possible higher doses for children under 6 years.

Chemotherapy-Induced Anemia (Adult Cancer Patients):

• Initial dose: 500 µg once every three weeks, or 2.25 µg/kg once weekly.

Administration of Embardia 60mcg/0.3ml Injection

• Do not shake or freeze.
• Protect from light and inspect for particulates or discoloration before use.
• Do not dilute or mix with other drug solutions.

Interaction

No formal drug interaction studies have been conducted with Darbepoetin alfa.

Contraindications

• Uncontrolled hypertension
• Pure Red Cell Aplasia (PRCA)
• History of serious allergic reactions to Darbepoetin alfa

Pregnancy & Lactation

• Pregnancy Category C: Not enough data is available to determine potential harm to a fetus.
• It is not known whether Darbepoetin alfa is excreted in breast milk; use with caution in nursing mothers.

Precautions & Warnings

• Use with caution in patients with controlled hypertension, seizures, or a history of PRCA or allergies to Darbepoetin alfa.
• Monitor hemoglobin levels closely to avoid exceeding recommended levels.
