- With food: Food reduces iron bioavailability after Femotol administration.

- Hemochromatosis or iron overload syndromes.
- Repeated blood transfusions.

Pregnancy:
- Ferric maltol is not systemically absorbed as an intact complex, so fetal exposure is unlikely.
- Animal studies show no adverse developmental outcomes at doses 13-32 times the recommended human dose.
- Untreated iron deficiency anemia (IDA) increases risks of postpartum anemia, preterm delivery, and low birth weight.
Lactation:
- Lack of data regarding excretion in human milk; breastfeeding is not expected to expose the child to the drug.

- Avoid use during active inflammatory bowel disease (IBD) flare-ups due to increased gastrointestinal inflammation risk.
- Do not administer in cases of iron overload or patients receiving intravenous iron.
- Assess and monitor iron parameters before and during therapy.
- Accidental overdose is a leading cause of fatal poisoning in children under 6 years. Immediate treatment is necessary in case of overdose.
