✔️ Indication of Tofacent 10mg Tablet

• Rheumatoid Arthritis: Tofacent is prescribed for adults with moderately to severely active rheumatoid arthritis who have not responded adequately to or cannot tolerate methotrexate. It can be used alone or with methotrexate or other nonbiologic DMARDs.
• Psoriatic Arthritis: Tofacent is used for adults with active psoriatic arthritis who have had an inadequate response to or cannot tolerate methotrexate or other DMARDs.
• Ulcerative Colitis: Tofacent treats adults with moderately to severely active ulcerative colitis.

✔️ Pharmacology

Tofacitinib is a partial and reversible Janus kinase (JAK) inhibitor. It interferes with the body's response to cytokine signals by preventing the activation of the JAK-STAT pathway, thereby reducing inflammation and immune responses. This mechanism helps decrease tissue inflammation and joint damage in rheumatoid arthritis.

✔️ Dosage & Administration of Tofacent 10mg Tablet

• Rheumatoid Arthritis:
  • Standard: 5 mg twice daily or 11 mg once daily.
  • For patients with moderate/severe renal or moderate hepatic impairment: 5 mg once daily.
• Psoriatic Arthritis:
  • Standard: 5 mg twice daily or 11 mg once daily.
  • For patients with moderate/severe renal or moderate hepatic impairment: 5 mg once daily.
• Ulcerative Colitis:
  • Initial: 10 mg twice daily for at least 8 weeks.
  • Maintenance: 5 or 10 mg twice daily. Discontinue if no adequate therapeutic benefit is achieved after 16 weeks of 10 mg twice daily. Use the lowest effective dose to maintain response.
• Pediatric Use: Safety and effectiveness not established.
• Geriatric Use: Higher frequency of serious infections in patients 65 years and older. Use caution.
• Renal/Hepatic Impairment: Half the total daily dosage for patients with moderate/severe renal or moderate hepatic impairment.

• Potent CYP3A4 Inhibitors (e.g., Ketoconazole): Dose adjustment to 5 mg once daily.
• Medications that moderately inhibit CYP3A4 and potently inhibit CYP2C19 (e.g., fluconazole): Dose adjustment to 5 mg once daily.
• Potent CYP Inducers (e.g., Rifampin): May reduce clinical response.
