How does Arofil 400 SR Tablet work?

Arofil 400 SR is a bronchodilator. It works by relaxing the muscles of the airways and widening the airways. This makes breathing easier.

Side Effects of Arofil 400 SR Tablet

• Common side effects at normal doses are rare and include:
  • Gastrointestinal discomfort, headache, nausea, insomnia, and hypotension.
  • CNS stimulation and diuresis (more common in children).

• You have been prescribed Arofil 400 SR for the prevention and treatment of asthma and chronic obstructive pulmonary disease (COPD).
• It should be taken at the same time each day, preferably in the evening after food.
• It does not work right away and should not be used to relieve sudden breathing problems. Always keep a fast-acting (rescue) inhaler with you.
• Your doctor may take regular blood tests to monitor potassium levels and the level of this medicine in your body.
• Notify your doctor if you have ever been diagnosed with kidney, liver, or heart disease, or if you have a smoking history. Your dose may need to be adjusted.
• Do not discontinue use without consulting your doctor, even if you feel better.

Indication of Arofil 400 SR Tablet

Arofil 400 SR is used to treat and prevent symptoms of asthma and chronic obstructive pulmonary disorder (a lung disorder in which the flow of air to the lung is blocked). 

Pharmacology

Theophylline is a bronchodilator belonging to the Methylxanthine class. It relaxes smooth muscles in the airways and suppresses the airway's response to stimuli, making it useful in treating conditions like asthma and COPD. Theophylline also enhances diaphragmatic muscle contraction, improving breathing in patients. The half-life of Theophylline varies between 3 to 9 hours, depending on several factors, such as smoking, age, and co-existing conditions.

Dosage & Administration of Arofil 400 SR Tablet

Recommended Maintenance Dosage:

• 16 years or older: 10 mg/kg/day (Max: 900 mg/day).
• 12-15 years: 13 mg/kg/day.
• 9-11 years: 16 mg/kg/day.
• 1-8 years: 24 mg/kg/day.
• 6 months year: 12-18 mg/kg/day.
• 1-6 months: 10 mg/kg/day.

The dose of Theophylline is individualized based on peak serum Theophylline concentration to balance effectiveness and minimize adverse effects. Sustained-release formulations are usually administered every 12 hours in adults, though children may require dosing every 8 hours due to faster metabolism.

Interaction

Avoid concurrent use with xanthine derivatives.
Drugs that increase the clearance of Arofil SR:

• Barbiturates, carbamazepine, lithium, phenytoin, rifampicin, and sulphinpyrazone. Drugs that reduce the clearance of Arofil SR:
• Allopurinol, cimetidine, ciprofloxacin, corticosteroids, erythromycin, frusemide, isoprenaline, oral contraceptives, and thiabendazole.

Hypokalemia risk: Arofil SR can worsen hypokalemia when used with beta-2-agonists, steroids, diuretics, and during hypoxia. Serum potassium levels should be monitored in such cases.

Contraindications

Hypersensitivity to Theophylline or any component of Arofil SR.

Pregnancy & Lactation

• It is not known whether Theophylline causes fetal harm; it should only be used during pregnancy if needed.
• Theophylline is excreted in breast milk and may cause irritability or mild toxicity in nursing infants. Serious adverse effects are unlikely unless toxic serum levels are present in the mother.

Precautions & Warnings

• Consider possible drug interactions and physiological conditions that could alter Arofil SR clearance.
• Start with a low dose and increase gradually.
• Pediatric Use: Theophylline clearance is very low in neonates, requiring careful dosage selection and monitoring.
