Pharmacology

Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT1). Cysteinyl leukotrienes (LTC4, LTD4, LTE4), products of arachidonic acid metabolism, are released from various cells, including mast cells and eosinophils. These leukotrienes and receptor occupation are linked to asthma and allergic rhinitis pathophysiology, contributing to airway edema, smooth muscle contraction, and altered cellular activity in the inflammatory process.

Dosage & Administration:

• Adults and adolescents (15 years and older) with asthma or seasonal allergic rhinitis: 10 mg tablet once daily.
• Pediatric patients (6 to 14 years) with asthma or seasonal allergic rhinitis: 5 mg tablet once daily.
• Pediatric patients (2 to 5 years) with asthma or seasonal allergic rhinitis: 4 mg tablet once daily.
• Pediatric patients (6 months to 5 years) with asthma: 4 mg oral granules once daily, either directly or mixed with cold water or soft food.

Use in Pediatric Patients: Safety and efficacy are established in pediatric patients 6 months to 14 years old, similar to adults.

Hepatic Insufficiency: No dosage adjustment for mild-to-moderate hepatic insufficiency.

Renal Insufficiency: No dosage adjustment needed.

Elderly Use: Pharmacokinetics and oral bioavailability are similar in elderly and younger adults. The plasma half-life is slightly longer in the elderly, but no dosage adjustment is necessary.

Interaction

Arokast can be used with other asthma therapies without increased adverse reactions. It does not significantly affect the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, or warfarin. It was also used with various commonly prescribed drugs, such as thyroid hormones and NSAIDs, without adverse interactions. Phenobarbital reduces Arokast's AUC by 40%, but no dosage adjustment is needed. Monitor when co-administered with potent cytochrome P450 enzyme inducers like phenobarbital or rifampin.

Contraindications

Montelukast is contraindicated in patients hypersensitive to any of its components.

Pregnancy & Lactation

Montelukast crosses the placenta in animal studies. There are no adequate studies in pregnant women, so use only if clearly needed. As many drugs are excreted in human milk, caution is advised when administering to nursing mothers.

Precautions & Warnings

Arokast is not for acute asthma attacks. Patients should have rescue medication available. Therapy can continue during acute exacerbations, but it should not replace corticosteroids abruptly. Not recommended as monotherapy for exercise-induced bronchospasm. Patients with aspirin sensitivity should avoid aspirin or NSAIDs. Arokast improves airway function in aspirin-sensitive asthmatics but does not prevent bronchoconstrictor responses to these drugs.
