How does Monocast 10mg Tablet work?

Monocast 10 mg selectively and actively inhibits the CysLT1 receptor, preventing cysteinyl leukotrienes from causing airway edema, smooth muscle contraction, and inflammatory changes associated with asthma and allergic rhinitis.

Side Effects of Monocast 10mg Tablet

Common side effects include:

• Upper respiratory tract infection, diarrhea, fever, gastrointestinal pain, headache, nausea, and vomiting.

Uncommon side effects include:

• Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, hemorrhage, irritability, malaise, muscle complaints, edema, seizure, and sleep disturbances.

Rare side effects include:

• Pulmonary eosinophilia, angioedema, decreased attention, disorientation, erythema nodosum, hallucinations, hepatic issues, memory loss, palpitations, suicidal thoughts, and tremors.

Indication of Monocast 10mg Tablet

Monocast 10 mg is indicated for:

• Prophylaxis and chronic treatment of asthma
• Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
• Relief of symptoms of Allergic Rhinitis (AR), both seasonal and perennial

Pharmacology

Monocast 10 mg operates by inhibiting the cysteinyl leukotriene receptor (CysLT1). The metabolism of arachidonic acid produces cysteinyl leukotrienes (LTC4, LTD4, and LTE4), which are released from cells like mast cells and eosinophils. These leukotrienes bind to receptors located in the human airway. Monocast 10 mg specifically targets and blocks these receptors without any agonist activity, thereby inhibiting the physiological effects of LTD4.

Dosage & Administration:

Adults and Adolescents (15 years and older) with Asthma or Seasonal Allergic Rhinitis:

• Dosage: 10 mg tablet once daily.

Pediatric Patients with Asthma or Seasonal Allergic Rhinitis:

• Ages 6 to 14 years: 5 mg tablet once daily.
• Ages 2 to 5 years: 4 mg tablet once daily.
• Ages 6 months to 5 years: 4 mg oral granules once daily. These can be administered directly in the mouth or mixed with cold water or soft food at room temperature.

Special Populations:

• Hepatic Insufficiency: No dosage adjustment is needed for mild-to-moderate hepatic insufficiency.
• Renal Insufficiency: No dosage adjustment is required.
• Elderly: Pharmacokinetic profiles and oral bioavailability are similar to younger adults. No dosage adjustment is necessary.

Interaction

Monocast 10 mg can be used with other common asthma treatments without increasing the risk of adverse effects. Studies show no significant interaction with drugs like theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin. Even though detailed interaction studies weren't conducted, no harmful interactions were observed when used with thyroid hormones, hypnotics, benzodiazepines, NSAIDs, and decongestants. Phenobarbital reduced the drug’s AUC by about 40%, but no dosage adjustment is recommended. Clinical monitoring is advised when used with potent cytochrome P450 enzyme inducers like phenobarbital or rifampin.

Contraindications

Patients with hypersensitivity to any ingredient in Monocast 10 mg should not use it.

Pregnancy & Lactation

Classified as pregnancy category B. While animal studies show the drug crosses the placenta, there are no well-controlled studies in pregnant humans. Some evidence suggests secretion in breast milk, but the implications are unclear. Caution is advised before using during pregnancy or nursing.

Precautions & Warnings

• Not recommended for reversing bronchospasm in acute asthma attacks or status asthmaticus.
• Patients should have appropriate rescue medication accessible.
• Not to be abruptly substituted for inhaled or oral corticosteroids.
• Exercise-induced bronchospasm should not be treated with a single dose.
• Patients with aspirin sensitivity should avoid aspirin and NSAIDs, as the drug does not reduce bronchoconstrictor responses to these medications.
