Indications
The two medications that make up Seroxyn HFA 25/125 work together to widen the airways and improve breathing. It is used to treat chronic obstructive pulmonary disease and asthma (wheezing and shortness of breath) (a lung disorder in which the flow of air to the lungs is blocked). How frequently you should take your inhaler will be determined by your doctor. This is recommended for patients who are not adequately controlled with inhaled corticosteroids and 'as needed' inhaled short-acting 2-agonist or who are already adequately controlled on both inhaled corticosteroid and long-acting 2-agonist when a combination product (long-acting 2-agonist and inhaled corticosteroid) is appropriate.
Pharmacology
A long-acting, selective beta-2 agonist called salmeterol Xinafoate is used to treat diffuse airway obstructions like asthma. A corticosteroid called fluticasone propionate predominantly functions as a glucocorticoid. Fluticasone Propionate is reported to only affect the lungs topically at typical doses, with no systemic side effects.
Dosage & Administration
Inhalation Aerosol:
Adults and teenagers For patients 12 years of age and older, the recommended dosage is two puffs of 25 mg salmeterol and 50 mg fluticasone propionate, two puffs of 25 mg salmeterol and 125 mg fluticasone propionate, or two puffs of 25 mg salmeterol and 250 mg fluticasone propionate twice daily.
Children (4–12 years): 2 puffs each twice day each containing 25 g of salmeterol and 50 g of fluticasone propionate.
Inhalation Powder in Capsule (For Asthma):
Salmeterol 50 mg & Fluticasone 100 mg or Salmeterol 50 mg & Fluticasone 250 mg twice daily for adults and adolescents (12 years and older) (morning and evening, approximately 12 hours apart).
For individuals 12 years of age and older, the starting doses of salmeterol 50 mg, fluticasone 100 mg, and salmeterol 50 mg, fluticasone 250 mg are dependent on the severity of the patient's asthma.
The maximum dose that is advised is Fluticasone 500 mg twice daily and Salmeterol 50 mg.
Salmeterol 50 mg & Fluticasone 100 mg twice daily are the recommended dosages for children (4–11 years old) with uncontrolled asthma who are not taking an inhaled corticosteroid (morning and evening, approximately 12 hours apart).
Salmeterol 50 g and fluticasone 250 g inhalation powder in capsules (for COPD) twice a day (morning and evening, approximately 12 hours apart). After each inhalation, mouthwash is suggested.
The Maxhaler (For Asthma) contains pre-dispensed inhalation powder in 60 evenly spaced blisters on a foil strip. It is a molded plastic device. Patients should be informed that Maxhaler must be taken consistently for best results, even in the absence of symptoms.
Adults and Adolescents (12 years and older)-
50/100 Maxhaler: One Inhalation twice daily
50/250 Maxhaler: One Inhalation twice daily
50/500 Maxhaler: One Inhalation twice daily
Children (4 years and older)-
50/100 Maxhaler: Two daily inhalations. Fluticasone propionate given by this Maxhaler in children may not exceed the maximum allowed dose of 100 ugs twice daily. There are no statistics on the usage of this Maxhaler in children under the age of 4 available.
Inhalation Powder in Maxhaler (For COPD):
Maxhaler: One Inhalation twice daily
Special patient populations: Patients who are elderly or have renal impairment do not require dose adjustments. For use in patients with hepatic impairment, there are no data available.

Interaction
Both non-selective and selective -blockers ought to be avoided in asthma patients unless medically necessary. Due to the very low plasma concentrations obtained after inhalation treatment, clinically significant drug interactions are unlikely. There is a chance of enhanced systemic exposure to Fluticasone Propionate when known strong CYP3A4 inhibitors (such as ketoconazole and ritonavir) are also used.
Contraindications

Drugs should only be administered during pregnancy and lactation if the predicted benefit to the mother outweighs any potential danger to the fetes or child. Salmeterol Xinafoate and Fluticasone Propionate have insufficient expertise in human pregnancy and lactation. There is no information about human breast milk.
Precautions & Warnings
Consideration should be given to additional corticosteroid therapies, and to including the administration of antibiotics if an infection is present. As with all inhaled medication containing corticosteroids, this preparation should be administered with caution in patients with active or quiescent pulmonary tuberculosis. This preparation should be administered with caution in patients with thyrotoxicosis.
