Oncological Uses (Supportive Therapy):
Tranexamic acid operates through competitive inhibition of the activation of plasminogen. Plasminogen is activated to plasmin, an enzyme responsible for breaking down fibrin clots, fibrinogen, and other plasma proteins. The inhibition occurs through binding to the kringle domain of plasminogen.
The primary actions include:
By interfering with the fibrinolytic process, tranexamic acid helps maintain hemostasis by preventing excessive breakdown of blood clots and fibrin components.
Pharmacokinetics:
Administration: May be taken with or without food.
Potentially fatal interactions with estrogens, Factor IX complex concentrates, or anti-inhibitor coagulant concentrates. Increased risk of fatal thrombotic complications with tretinoin in acute promyelocytic leukemia.
Severe renal failure, active intravascular clotting, thromboembolic disease, color vision disorders, subarachnoid bleeding.
Tranexamic acid crosses the placenta. Clinical experience of use in pregnant women is limited. Animal studies have not supplied any evidence of an increased incidence of fetal damage. Tranexamic acid is excreted into breast milk, but it is not likely to influence the child at therapeutic doses.
Mild to moderate renal impairment, irregular menstrual bleeding, previous history of thromboembolic disease, and haematuria. Monitor closely in disseminated intravascular coagulation. Monitor LFT and eye examination regularly during long-term use. Discontinue if the disturbance in color vision occurs. Avoid IV in rate >1 ml/minute due to the risk of hypotension. Pregnancy, lactation.
Store in a dry place at 15-30°C, away from light and keep out of children's reach.
