Oxaliplatin undergoes nonenzymatic conversion in physiologic solutions, forming active derivatives that bind to DNA, leading to cross-linking. These crosslinks, between guanine bases and other nucleotides, prevent DNA replication and transcription, causing cytotoxic effects that are not dependent on the cell cycle. Oxaliplatin has shown antitumor activity in colon carcinoma and, in combination with 5-fluorouracil, has enhanced antiproliferative effects compared to either compound alone in various tumor models (colon, mammary, leukemia).

✔️ Eloxatin, used in combination with infusional 5-fluorouracil/folinic acid, is indicated for:
- Adjuvant treatment of stage III colon cancer in patients who have undergone complete resection of the primary tumor.
- Treatment of advanced colorectal cancer.

✔️ Dosage & Administration of
Administer oxaliplatin with 5-fluorouracil/folinic acid every 2 weeks. For advanced disease, continue treatment until disease progression or unacceptable toxicity. For adjuvant use, the treatment duration should be 6 months (12 cycles).
Day 1:
- Oxaliplatin 85 mg/m² IV infusion in 250-500 mL 5% Dextrose, followed by folinic acid 200 mg/m² IV infusion over 120 minutes.
- Then, administer 5-fluorouracil 400 mg/m² IV bolus (2-4 minutes), followed by 5-fluorouracil 600 mg/m² IV infusion in 500 mL 5% Dextrose for 22 hours.
Day 2:
- Folinic acid 200 mg/m² IV infusion over 120 minutes, followed by 5-fluorouracil 400 mg/m² IV bolus and 600 mg/m² IV infusion in 500 mL 5% Dextrose for 22 hours.

✔️ Uses of Eloxatin
- Treatment of Colon and rectal cancer
✔️ Common side effects (incidence 40%) include:
- Peripheral sensory neuropathy, neutropenia, thrombocytopenia, anemia, nausea
- Increased liver enzymes (transaminases, alkaline phosphatase), diarrhea, vomiting, fatigue, stomatitis.
Other serious side effects may also occur.
✔️ Interactions
There are no specific cytochrome P-450 interaction studies. No pharmacokinetic interaction was observed between 85 mg/m² Eloxatin and 5-fluorouracil/folinic acid when dosed every 2 weeks. Higher doses (130 mg/m² every 3 weeks) may increase 5-fluorouracil plasma concentrations by about 20%. The clearance of platinum-containing compounds may be affected by nephrotoxic drugs.
✔️ Contraindications
Eloxatin is contraindicated in patients with a known allergy to oxaliplatin or other platinum-based compounds.
✔️ Overdose Effects
There is no known antidote for overdose. Overdose symptoms may include thrombocytopenia, hypersensitivity reactions, myelosuppression, nausea, vomiting, diarrhea, neurotoxicity, and severe respiratory or cardiac symptoms. Supportive treatment should be administered if an overdose occurs. The maximum single infusion dose recorded is 825 mg.
✔️ Pregnancy & Lactation
Pregnancy Category D. Eloxatin may cause fetal harm and should be avoided during pregnancy. Women of childbearing potential should avoid pregnancy during treatment. It is unknown if oxaliplatin is excreted in breast milk; consider discontinuing breastfeeding or the drug based on its importance to the mother.

- Allergic Reactions: Severe allergic reactions, including anaphylaxis, can occur and require discontinuation of therapy.
- Neurological Toxicity: Monitor for neurological effects, particularly if combined with other neurotoxic drugs.
- Pulmonary Toxicity: Eloxatin may cause pulmonary fibrosis, which can be fatal. Discontinue if respiratory issues arise.
- Hepatotoxicity: Liver function should be monitored for signs of liver sinusoidal obstruction syndrome.
- Cardiovascular Toxicity: Monitor for QT prolongation and arrhythmias, especially in patients with heart issues or electrolyte abnormalities.
