Oxaliplatin undergoes nonenzymatic conversion in physiologic solutions, forming active derivatives that bind to DNA, leading to cross-linking. These crosslinks, between guanine bases and other nucleotides, prevent DNA replication and transcription, causing cytotoxic effects that are not dependent on the cell cycle. Oxaliplatin has shown antitumor activity in colon carcinoma and, in combination with 5-fluorouracil, has enhanced antiproliferative effects compared to either compound alone in various tumor models (colon, mammary, leukemia).

Administer oxaliplatin with 5-fluorouracil/folinic acid every 2 weeks. For advanced disease, continue treatment until disease progression or unacceptable toxicity. For adjuvant use, the treatment duration should be 6 months (12 cycles).

There are no specific cytochrome P-450 interaction studies. No pharmacokinetic interaction was observed between 85 mg/m² Eloxatin and 5-fluorouracil/folinic acid when dosed every 2 weeks. Higher doses (130 mg/m² every 3 weeks) may increase 5-fluorouracil plasma concentrations by about 20%. The clearance of platinum-containing compounds may be affected by nephrotoxic drugs.
Eloxatin is contraindicated in patients with a known allergy to oxaliplatin or other platinum-based compounds.
There is no known antidote for overdose. Overdose symptoms may include thrombocytopenia, hypersensitivity reactions, myelosuppression, nausea, vomiting, diarrhea, neurotoxicity, and severe respiratory or cardiac symptoms. Supportive treatment should be administered if an overdose occurs. The maximum single infusion dose recorded is 825 mg.
Pregnancy Category D. Eloxatin may cause fetal harm and should be avoided during pregnancy. Women of childbearing potential should avoid pregnancy during treatment. It is unknown if oxaliplatin is excreted in breast milk; consider discontinuing breastfeeding or the drug based on its importance to the mother.
