Xitabin is used for the treatment of: Adjuvant colon cancer (Dukes' C colon cancer). Metastatic colorectal cancer (as first-line monotherapy when fluoropyrimidine therapy is preferred). Metastatic breast cancer (in combination with docetaxel after failure of prior anthracycline-containing therapy). As monotherapy in patients resistant to both paclitaxel and anthracycline-containing regimens.

Monitor for signs of coagulopathy (bleeding), diarrhea, cardiotoxicity (especially in patients with a history of coronary artery disease), and mucocutaneous toxicity. Patients with low or absent Dihydropyrimidine Dehydrogenase (DPD) activity should not use Xitabin due to the risk of severe reactions. Stop treatment if dehydration or renal failure occurs, or if hyperbilirubinemia or severe hand-and-foot syndrome develops.

Overdose may lead to symptoms such as nausea, vomiting, diarrhea, mucositis, gastrointestinal irritation, bleeding, and bone marrow depression. Immediate medical attention should be sought for overdose management.

If you experience severe or persistent side effects such as diarrhea, mucositis, or rash, consult your doctor. In some cases, treatment may need to be stopped or adjusted.

Xitabin is not recommended for children under the age of 18 as safety and effectiveness have not been established.

Xitabin is a pregnancy category D medication, meaning it can cause fetal harm. It is not recommended during pregnancy or breastfeeding. Breastfeeding should be discontinued during treatment and for 2 weeks after the final dose.

Be cautious when taking Xitabin with anticoagulants, phenytoin, leucovorin, or CYP2C9 substrates. Regular monitoring and dose adjustments may be necessary.

Common side effects include abdominal pain, rash, dry or itchy skin, tiredness, loss of appetite, diarrhea, vomiting, nausea, stomatitis, hand-and-foot skin reaction, fever, infection, and chest pain.

The recommended dose is 1250 mg/m² twice daily for 2 weeks followed by a one-week rest period in 3-week cycles. It should be taken within 30 minutes after meals (breakfast and dinner) and swallowed whole with water. Do not crush or cut the tablets.
