How does Prulax 1 work?

It acts on the muscle walls of the gut to increase bowel motility, helping to restore normal bowel function and alleviate constipation.

Indication of Prulax 1

Prucalopride is used in the treatment of constipation. It helps in relieving constipation that is long-lasting or keeps coming back.

Pharmacology

Prucalopride acts as a selective stimulator of the 5-HT4 receptors while having no interaction with hERG channel or 5-HT1 receptors which reduces significantly the cardiovascular risk found in other similar drugs. 5-HT4 receptors can be found throughout the gastrointestinal tract primarily in smooth muscle cells, enterochromaffin cells, and myenteric plexus. Its activation produces the release of acetylcholine which is the major excitatory neurotransmitter in the GI tract. Hence, prucalopride stimulates motility by interacting specifically with 5-HT4 receptors in the GI tract which causes a release of acetylcholine and further contraction of the muscle layer of the colon and relaxation of the circular muscle layer leading to the propulsion of luminal content.

Dosage & Administration of Prulax 1

• Take Prucalopride as long as prescribed by your doctor.
• Note: It's important to follow your doctor's instructions regarding dosage.
• Renal Impairment:
  • Severe: 1 mg once daily (GFR <30 ml/min/1.73 m²).
  • Mild to Moderate: No dose adjustment required.
• Hepatic Impairment:
  • Severe: Start with 1 mg once daily, may increase to 2 mg if tolerated.
  • Mild to Moderate: No dose adjustment required.

Contraindications

• They are contraindicated in individuals who are hypersensitive to prucalopride or any of its excipients.
• It is not recommended for patients with renal impairment requiring dialysis.

Pregnancy & Lactation

• Pregnancy: Not recommended for use during pregnancy. Women of childbearing potential should use effective contraception during treatment.
• Lactation: Not recommended for breastfeeding due to a lack of human data, although animal studies have not shown direct or indirect harmful effects.

Precautions & Warnings

• Renal Excretion: Prucalopride is mainly eliminated through renal excretion.
  • Recommended Dose for Severe Renal Impairment: 1 mg once daily.
• Hepatic Impairment: Caution is advised in patients with severe hepatic impairment (Child-Pugh class C). Start with 1 mg once daily, which may be increased to 2 mg if well tolerated.
• Diarrhea: Severe diarrhea may reduce the efficacy of oral contraceptives. An additional contraceptive method is recommended to prevent failure of oral contraception.
• Lactose Content: Contains lactose; unsuitable for patients with hereditary galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption.

Overdose Effects of Prulax 1

•  Symptoms: Overdose may lead to symptoms such as:
  • Exaggeration of known effects (headache, nausea, diarrhea).

Management:

• Specific treatment for overdose is not available. Management should be symptomatic and supportive. Correct any electrolyte disturbances due to fluid loss from diarrhea or vomiting.

Adverse reactions are more common at the start of therapy and typically resolve within a few days. Most events are mild to moderate in intensity.
