Oral electrolytes preparations

  • This saline contains essential electrolytes and carbohydrates (in the form of processed rice) vital for dehydrating patients in conditions such as cholera, acute diarrhea, vomiting, and excessive sweating.
  • It effectively replenishes lost electrolytes in the body, including Chloride ions, extracellular sodium ions, and intracellular potassium ions.
  • The dose depends on the severity of the dehydrating conditions of the patients. Recommended dose after each watery stool are as follows:
    • 6 months to 2 years: 50 ml to 100 ml (10 to 20 Teaspoonfuls)
    • 2 years to 9 years: 100 ml to 200 ml (20 to 40 teaspoonfuls)
    • 10 years and above 250 ml to 500 ml (1 to 2 glassfuls)

For 250 ml water:

✔️ Indication of Rice ORS Oral Powder
  • This saline is indicated in:
    • Acute fluid and electrolyte loss conditions such as cholera, acute diarrhea, and vomiting
    • Dehydration
    • Severely low concentrations of salts in the blood (severe electrolyte depletion)
✔️ Pharmacology
  • Potassium chloride: Plays an active role in nerve impulse conduction, muscle contraction, maintenance of renal function, acid-base balance, carbohydrate metabolism, and gastric secretion.
  • Sodium chloride: Important in electrolyte and fluid balance, osmotic pressure control, and water distribution.
  • Dextrose: Used as a source of calories and water for hydration, reduces loss of body protein and nitrogen, promotes glycogen deposition in the liver, and stimulates potassium uptake by cells when used with insulin.
✔️ Dosage of Rice ORS Oral Powder

Adult Dose:

  • After each loose stool or vomiting, administer 200-400 ml of prepared saline.
  • The daily dose should be equivalent to patients' fluid requirement for maintenance and replenishment of losses.
  • During saline therapy, normal food should be continued in the case of adults.

Child Dose:

  • Children above 10 years: After each loose stool or vomiting, administer 200-400 ml of prepared saline.
  • Children 2 to 10 years: After each loose stool or vomiting, administer 100-200 ml of prepared oral saline.
  • Children less than 2 years: After each loose stool or vomiting, administer 10 to 20 spoonfuls (50-100 ml) of prepared saline.
  • The daily dose should be equivalent to patients' fluid requirement for maintenance and replenishment of losses.
  • During saline therapy, mothers should not stop breastfeeding their children.

Preparation for 250 ml Saline:

  • Add 250 ml (one glassful) of hot boiled water into a jar.
  • Then add a full sachet of Rice ORS (for 250 ml water) powder into the jar.
  • Mix the powder well in the water until a milky white solution appears.
  • Give the solution to the patient to drink as per the dose.

Preparation for 500 ml Saline:

  • Add 500 ml hot boiled water (two glassfuls) into a jar.
  • Then add a full sachet of Rice ORS (for 500 ml water) powder into the jar.
  • Mix the powder well in the water until a milky white solution appears.
  • Give the solution to the patient to drink as per the dose.

  • There is no known drug interaction reported.
✔️ Contraindications

Not to be used in:

  • Kidney failure resulting in diminished production of urine (oliguria)
  • Kidney failure, preventing the production of urine (anuria)
  • Obstruction of the stomach or intestines
  • Reduced blood flow to vital internal organs (shock)
  • Severe and continuous vomiting (intractable vomiting)
  • Severe dehydration
  • Severe diarrhea in infants

Recommended in pregnancy & lactation, as there are no known harmful effects when this medicine is used.

✔️ Precautions & Warnings
  • Precautions should be taken in case of significant overdose, especially for the following patients:
    • Children less than 1 year of age
    • Patients with an imbalance of salt concentrations in the blood (electrolyte imbalance)
    • Patients with severely decreased kidney and liver function
  • Depressed renal function, severe continuing diarrhea, or other critical fluid losses may need supplementation with parenteral fluids along with oral saline.
  • Reconstituted saline should be used within 6 hours.

Consult your doctor before taking corticosteroids or corticotropin as it may increase the risk of sodium fluid retention.

The risk for hyponatremia is increased in the elderly, pediatric patients, postoperative patients, and patients who are taking drugs that may increase the risk of hyponatremia. Close clinical monitoring in such patients is advised.

Rice ORS Oral Powder may increase the risk of hyperkalemia (high level of potassium in the blood). It should be avoided in patients with or at risk for hyperkalemia.
