How does Valex CR 300 work?

Valex CR 300 is a combination of two antiepileptic medications: Sodium Valproate and Valproic Acid. Each enteric-coated controlled-release tablet contains:

• Sodium Valproate: 199.8 mg
• Valproic Acid: 87 mg

These two components work synergistically to manage seizures or fit by reducing abnormal and excessive activity in the brain's nerve cells. By stabilizing the electrical activity in the brain, they help control various types of seizures.

Side Effects of Valex CR 300

Common side effects include anorexia, nausea, and vomiting, which are minimized with enteric-coated tablets. Central nervous system effects include sedation, ataxia, and tremor, typically resolving with dose reduction. Occasionally, rash, alopecia, and increased appetite are observed. Liver enzyme elevations are common and usually asymptomatic, but rare cases of fatal fulminant hepatitis can occur. Hepatic injury, acute pancreatitis, and hyperammonemia are also associated risks, particularly in young children with other medical conditions or multiple antiepileptic treatments.

Pharmacology

Sodium Valproate has anti-epileptic properties effective against various seizure types. Its mechanism is thought to involve increasing brain levels of gamma-aminobutyric acid (GABA).

Dosage of Valex CR 300

Oral:

• Epilepsy:
  • Adults: Start with 600 mg daily in two divided doses, increasing by 200 mg/day every three days up to 2.5 g daily until seizures are controlled. The maintenance dose is usually 1-2 g daily (20-30 mg/kg daily).
  • Children over 20 kg: Start with 400 mg daily in divided doses, increasing as needed (usually 20-30 mg/kg daily), up to a maximum of 35 mg/kg daily.
  • Children up to 20 kg: Start with 20 mg/kg daily in divided doses.
• Febrile Convulsions: 20-30 mg/kg/day in three divided doses.
• Bipolar Disorder: Start with 600 mg daily (20-30 mg/kg/day) in 2-3 divided doses. The maintenance dose is 1000-2000 mg daily.
• Migraine Prophylaxis: 400-600 mg daily, some may need up to 1000-1500 mg daily.

Administration of Valex CR 300

• Take this medicine in the dose and duration as advised by your doctor.

• Do not chew, crush, or break it.

Interaction

Valex CR Valproate can inhibit the metabolism of various drugs, notably Phenobarbital, Phenytoin, Warfarin, and Aspirin.

Contraindications

Contraindicated in patients with known hypersensitivity to the drug or liver dysfunction. Restricted use during pregnancy and in women of childbearing potential.

Pregnancy & Lactation

Sodium Valproate crosses the placenta and has been associated with neural tube defects in newborns when taken during the first trimester. Pregnant women should consider serum alpha-fetoprotein testing. Sodium Valproate is excreted in breast milk, but breastfeeding is generally considered safe for the child.

Precautions & Warnings

• Monitor liver function before and during the first six months of therapy, especially in at-risk patients.
• Ensure no undue bleeding potential before starting and before major surgery.
• Exercise caution in renal impairment, pregnancy, breastfeeding, and systemic lupus erythematosus.
• Valex CR may cause false positives in urine ketone tests.
• Avoid sudden withdrawal.
• Not recommended for use during pregnancy and in women of childbearing potential.
