Uses:

• Epilepsy/Seizures
• Trigeminal neuralgia

Indications of Zeptol 200mg Tablet:

• Partial and secondary generalized tonic-clonic seizures
• Primary generalized tonic-clonic seizures
• Trigeminal neuralgia
• Prophylaxis of bipolar disorder

Pharmacology of Zeptol 200mg Tablet:

• Carbamazepine inhibits activity in the thalamic nucleus ventralis, limits synaptic propagation of excitatory impulses, and diminishes the summation of temporal stimuli leading to neuronal discharge by reducing the influx of Na ions across the cell membrane, among other processes. It increases the secretion of antidiuretic hormone (ADH) and enhances its ability to promote water reabsorption.

Dosage & Administration of Zeptol 200mg Tablet:

• For epilepsy:
  • Adults and children over 12 years of age: Initial dose of 200 mg b.i.d. for tablets and controlled-release tablets or 1 teaspoon q.i.d. for suspension (400 mg/day). Increase gradually at weekly intervals by adding up to 200 mg/day using a b.i.d, t.i.d., or q.i.d. regimen until optimal response is achieved.
  • Children 12-15 years of age: Dosage generally should not exceed 1000 mg daily, and 1200 mg daily in patients above 15 years of age. Maintenance dose is usually 800-1200 mg daily.
  • Children 6-12 years of age: Initial dose of 100 mg b.i.d. for tablets or controlled-release tablets, or 1/2 teaspoon q.i.d. for suspension (200 mg/day). Increase gradually at weekly intervals by adding up to 100 mg/day until optimal response is obtained. Maintenance dose is usually 400-800 mg daily.
  • Children under 6 years of age: Initial dose of 10-20 mg/kg/day b.i.d. or t.i.d. as tablets, or q.i.d. as suspension. Increase weekly to achieve optimal clinical response. Maintenance dose is ordinarily achieved below 35 mg/kg daily.
• For trigeminal neuralgia:
  • Initial dose of 100 mg b.i.d. for tablets or controlled-release tablets, or 1/2 teaspoon q.i.d. for suspension, for a total daily dose of 200 mg. Increase by up to 200 mg/day using increments of 100 mg every 12 hours for tablets or controlled-release tablets, or 50 mg q.i.d. for suspension, as needed to achieve pain relief. Maintenance dose can range from 400-1200 mg daily.

Interaction of Zeptol 200mg Tablet:

• Carbamazepine may interact with hepatic enzyme inducers like Carbamazepine and Phenytoin by enhancing Carbamazepine metabolism, potentially necessitating an increase in Zeptol 200mg Tablet dosage.

Contraindications of Zeptol 200mg Tablet:

• Zeptol 200mg Tablet should not be used in individuals allergic to its components or in the presence of certain conditions such as atrioventricular block, bone marrow depression, porphyrias, allergy to tricyclic antidepressants (TCAs), or recent use of monoamine oxidase inhibitor antidepressants (MAOI).

Side Effects of Zeptol 200mg Tablet:

• Common side effects include dizziness, drowsiness, ataxia, dry mouth, abdominal pain, nausea, vomiting, anorexia, leukopenia, proteinuria, bradycardia, heart failure, hypotension, erythema rash, and aplastic anemia. Increased sensitivity to sunlight may also occur.

Pregnancy & Lactation:

• Zeptol 200mg Tablet falls under pregnancy category D. Breastfeeding should be carefully considered due to the presence of carbamazepine and its metabolite in breast milk.

Precautions & Warnings:

• Zeptol 200mg Tablet can cause vertigo and drowsiness, so caution is advised during activities requiring mental alertness. Skin reactions and liver problems may occur, requiring immediate medical attention. It can also decrease blood levels of hormonal contraceptives, necessitating alternative contraceptive methods. Sudden withdrawal of treatment should be avoided.
