Omeprazole is a substituted benzimidazole and a potent proton pump inhibitor (PPI) that suppresses gastric acid secretion. It acts by selectively inhibiting the H⁺/K⁺-ATPase enzyme system in the parietal cells of the stomach — the final step in acid production.
After oral administration, acid suppression begins within one hour, reaches its peak in two hours, and may last up to 72 hours. Normal acid secretion resumes gradually within 3 to 5 days after discontinuation.

Benign gastric & duodenal ulcer:
20 mg once daily for 4 weeks (duodenal) or 8 weeks (gastric); may increase to 40 mg if necessary. Maintenance dose: 20 mg daily; prophylaxis: 10–20 mg daily.
NSAID-associated ulcers:
20 mg daily for 4 weeks (extend to 8 weeks if required). For prophylaxis, 20 mg once daily.
GERD:
20 mg daily for 4–8 weeks; in resistant cases, 40 mg daily for 8 weeks. Maintenance dose: 20 mg daily.
Long-term acid reflux management: 10–20 mg daily.

Generally mild and reversible. Reported adverse effects include:
Common: Headache, diarrhea, constipation, nausea, abdominal pain, flatulence.
Skin: Rash, urticaria, pruritus, photosensitivity, erythema multiforme, alopecia.
Neurological: Dizziness, somnolence, insomnia, vertigo, paresthesia, confusion, agitation, or hallucinations (mostly in severely ill patients).
Hepatic: Elevated liver enzymes, hepatitis (with or without jaundice), and rare hepatic failure.
Other: Arthralgia, myalgia, gynecomastia, edema, leukopenia, thrombocytopenia, anaphylaxis, bronchospasm, fever, malaise.
Studies show no harmful effects of omeprazole on pregnancy or fetal health; may be used during pregnancy if clearly indicated.
