Kinate Hydrochloride is a potent dopamine-2 antagonist and an acetylcholine esterase inhibitor. It increases acetylcholine concentrations by inhibiting dopamine D2 receptors and acetylcholinesterase. Higher acetylcholine increases Gl peristalsis, increases the lower esophageal sphincter pressure, stimulates gastric motility, accelerates gastric emptying, and improves gastro-duodenal coordination.
For adults, the typical oral dose is 50 mg of Itopride Hydrochloride, taken three times daily before meals. The dosage may be adjusted based on the patient’s age, condition, and response to therapy.
Elderly Use:
Elderly patients may have reduced physiological function, making them more susceptible to side effects. Therefore, they should be monitored closely, and if any adverse reactions occur, the dose should be reduced or treatment discontinued as necessary.
Pediatric Use:
The safety and effectiveness of Itopride Hydrochloride in children and adolescents under 18 years have not been established. Hence, its use in this age group is not recommended.
Caution is advised when Itopride Hydrochloride is used alongside anticholinergic agents such as tiquizium bromide, scopolamine butyl bromide, or timepidium bromide, as these may reduce the prokinetic (gastrointestinal motility-enhancing) effects of Itopride due to their opposing pharmacological actions.
Hypersensitivity to Itopride or any of its excipients.
Conditions where increased gastrointestinal motility is undesirable, such as gastrointestinal bleeding, mechanical obstruction, or perforation.
Shock and Anaphylactoid Reactions: Serious allergic reactions such as hypotension, breathing difficulty, throat swelling, urticaria, pallor, or excessive sweating may occur. If these symptoms appear, the drug should be stopped immediately and appropriate medical care provided.
Liver Dysfunction and Jaundice: Cases of hepatic impairment or jaundice with elevated AST (GOT), ALT (GPT), and γ-GTP levels have been reported. Regular monitoring is advised, and treatment should be discontinued if any abnormalities develop.
