How does Omenix 40 Capsule work?

A pump called the proton pump present in the stomach wall is responsible for the secretion of stomach acids. Omeprazole present in Omenix 40 binds to this proton pump in stomach cells, thus blocking acid secretion. Therefore, it is used in conditions where there is excessive acidity.

Severe Adverse Reactions

  • Spasms in the hands/feet or voice box

Extended use (typically over three years) may impair the body’s ability to absorb Vitamin B12, potentially leading to deficiency.

Gastroesophageal Reflux Disease

Omeprazole is used in the treatment of GERD (a condition where stomach acid and contents flow back and irritate the food pipe).

Zollinger-Ellison Syndrome

Omeprazole is used in the treatment of Zollinger-Ellison syndrome (a condition where the tumor in the upper part of the small intestine produces an excess amount of acid in the stomach.)

Duodenal Ulcer

Omeprazole is used in the treatment of Duodenal ulcers (a condition with open sores on the lining of the small intestine).

Helicobacter Pylori Infection

Omeprazole is used in the treatment of H.Pylori infection which is a most common cause of peptic ulcer caused by bacteria called Helicobacter pylori.

Erosive Esophagitis

Omeprazole is used in the treatment of Erosive Esophagitis which is an inflammation of the food pipe (Esophagus).

Pharmacology

Omeprazole, a substituted benzimidazole, acts as an inhibitor of gastric acid secretion. It works by blocking the hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in gastric parietal cells. After oral administration, the antisecretory effect begins within one hour, reaches its peak effect within two hours, and can inhibit secretion for up to 72 hours. Once the medication is discontinued, gastric acid secretion gradually returns to normal over 3 to 5 days.

Dosage & Administration: Omenix 40 Capsule

• Benign gastric and duodenal ulcers: 20 mg once daily for 4 weeks for duodenal ulcers, 8 weeks for gastric ulcers; in severe or recurrent cases, increase to 40 mg daily; maintenance dose for recurrent duodenal ulcers is 20 mg once daily; for prevention of relapse in duodenal ulcers, 10-20 mg daily.
• NSAID-associated gastric or duodenal ulcers: 20 mg once daily for 4 weeks; continue for an additional 4 weeks if not fully healed; use 20 mg once daily for prophylaxis in patients with a history of these ulcers.
• Gastroesophageal reflux disease: 20 mg once daily for 4 weeks; extend to 4-8 weeks if not fully healed; 40 mg once daily may be used for 8 weeks in refractory cases; maintenance dose is 20 mg once daily.
• Long-term management of acid reflux disease: 10-20 mg daily.
• Acid-related dyspepsia: 10-20 mg once daily for 2-4 weeks.
• Prophylaxis of acid aspiration: 40 mg the evening before surgery, then 40 mg 2-6 hours before surgery.
• Zollinger-Ellison syndrome: Start with 60 mg once daily; the usual range is 20-120 mg daily (for doses exceeding 80 mg, divide into 2 doses).
• Helicobacter pylori eradication in peptic ulcer disease: 20 mg twice daily with the following antimicrobial regimens: Amoxicillin 500 mg and Metronidazole 400 mg both three times a day for one week, or Clarithromycin 250 mg and Metronidazole 400 mg both twice a day for one week, or Amoxicillin 1 g and Clarithromycin 500 mg both twice a day for one week.
• Pediatric use in severe ulcerating reflux esophagitis (children over 1 year): If body weight is 10-20 kg, administer 10-20 mg once daily for 4-12 weeks; if body weight exceeds 20 kg, give 20-40 mg once daily for 4-12 weeks.
• Use in Children & Adolescents: The safety and effectiveness of Omeprazole have not been established in pediatric patients under 18 years of age.

Interaction of Omenix 40 Capsule

Omenix may reduce the absorption of ketoconazole due to decreased intragastric acidity, similar to other acid secretion inhibitors. It is metabolized in the liver via cytochrome P450, which can delay the elimination of diazepam, phenytoin, and warfarin. Monitoring of patients on warfarin or phenytoin is advised, and dosage adjustments may be necessary. However, concomitant treatment with Omenix 20 mg daily did not affect the blood concentration of phenytoin or coagulation time in patients on continuous warfarin therapy. The plasma concentrations of Omenix and clarithromycin may increase during co-administration, which is beneficial during H. pylori eradication. There is no evidence of interactions with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, or antacids. Alcohol or food does not affect the absorption of Omenix. There is no evidence of interaction with piroxicam, diclofenac, or naproxen, which is advantageous when patients need to continue these treatments. Simultaneous treatment with Omenix and digoxin may increase digoxin's bioavailability by 10% due to higher intragastric pH.

Contraindications: Omenix 40 Capsule

Omeprazole is contraindicated in patients with known hypersensitivity to it. Before initiating treatment with omeprazole in patients suspected of having a gastric ulcer, malignancy should be excluded, as treatment may mask symptoms and delay diagnosis.

Pregnancy & Lactation

Three prospective epidemiological studies indicate that Omeprazole does not adversely affect pregnancy or the health of the fetus or newborn. It can be used during pregnancy. However, there is limited information on whether Omeprazole passes into breast milk or its effects on neonates. Therefore, breastfeeding should be discontinued if Omeprazole is deemed essential.

Precautions & Warnings

Avoid using clopidogrel concurrently with Omenix, as it may reduce the effectiveness of clopidogrel. Observational studies suggest that proton pump inhibitor (PPI) therapy may be associated with an increased risk of osteoporosis-related fractures of the hip, wrist, or spine. Atrophic gastritis has occasionally been noted in gastric corpus biopsies from patients treated long-term with Omenix. The concomitant use of PPIs and methotrexate may lead to methotrexate toxicity.
