✅ Description:

Heart failure is a chronic condition where the heart doesn’t pump sufficient blood to other parts of the body, which is generally characterized by symptoms such as shortness of breath, fatigue, weakness, rapid or irregular heartbeat, and swelling in the legs, ankles, or feet.

Irregular heartbeat is a condition in which your heart beats irregularly, too fast or too slow. Symptoms of irregular heartbeat include chest pain, shortness of breath, and fast or slow heartbeat. 60 to 100 beats per minute is generally considered as normal heart rate.

Agoxin 0.25mg Tablet contains Digoxin which belongs to the group of medicines called Cardiac glycosides. It is used in the management of arrhythmias (irregular heartbeat, which causes the heart to skip a beat or beat at the wrong speed) and heart failure.

Agoxin 0.25mg Tablet is not recommended for use in patients with serious heart problems or irregular heartbeat due to cardiac glycoside intoxication).

Before taking Agoxin 0.25mg Tablet, inform your doctor if you have beriberi disease (which occurs due to vitamin B1 deficiency), severe lung disease, reduced renal function, or liver failure. Along with the management of a 0.25mg tablet of Agoxin, your doctor might advise you to take regular blood tests to determine the amount of digoxin in your blood and to check the status of kidney function, especially in patients with kidney disorders.

Agoxin 0.25mg Tablet should be used with caution in pregnant and breastfeeding women. Also, it should be used with caution in children and adolescents (below 18 years of age) and in elderly patients (aged 65 years and above).

✔️ Indication of Agoxin 0.25mg Tablet

Digoxin is used in the treatment of Heart failure and Arrhythmia.

✔️ Pharmacology

Digoxin is a cardiac glycoside used in the management of particularly atrial fibrillation and heart failure. The principal actions of digoxin are an increase in the force of myocardial contraction (positive inotropic activity and a reduction in the conductivity of the heart particularly in conduction through the atrioventricular node. Digoxin also has a direct action on the smooth vascular muscle and indirect effects mediated primarily by the autonomic nervous system, particularly by an increase in vagal activity.

✔️ The dosage guidelines for digoxin, administered orally, are as follows:

Oral Administration Dosage for Digoxin:

Rapid Digitalization:

  • Dosage: 1-1.5 mg in divided doses over 24 hours.
  • Purpose: This regimen is used when there is a need for a quick therapeutic effect, such as in acute situations.

Less Urgent Digitalization:

  • Dosage: 250-500 micrograms daily (higher doses may be divided).
  • Purpose: This regimen is used when a slower digitalization is appropriate, allowing the body to reach therapeutic levels of the drug gradually.

Maintenance Dose:

  • Dosage: 62.5-500 micrograms daily, with higher doses possibly divided.
  • Adjustments: The dose should be tailored based on renal function and the response of atrial fibrillation to heart rate control.

Usual Dosage Range:

  • General Range: 125-250 micrograms daily.
  • Elderly Patients: A lower dose, often within the range of 125-250 micrograms daily, may be appropriate due to reduced renal function and increased sensitivity to digoxin.

Considerations:

  1. Renal Function: Dosage adjustments are crucial for patients with impaired renal function to avoid toxicity.
  2. Atrial Fibrillation: The dosage should be individualized based on heart rate response in atrial fibrillation.
  3. Elderly Patients: They typically require lower doses due to decreased renal clearance and increased sensitivity to the drug.
✔️ Interaction

Drug-Drug Interaction: Agoxin 0.25mg Tablet may have interaction with medicines used to treat irregular heartbeat (adenosine, amiodarone, dolasetron, quinidine), cardiac stimulants (arbutamine), HIV/AIDS drugs (atazanavir, saquinavir), anticancer drugs (ceritinib, lapatinib), antibiotics (clarithromycin), immunosuppressants (fingolimod), antidepressants (flibanserin), antifungal drugs (itraconazole) and calcium supplements (calcium chloride, calcium gluceptate, calcium gluconate).

Drug-Food Interaction: Agoxin 0.25mg Tablet may interact with alcohol. Therefore, avoid alcohol intake while taking Agoxin 0.25mg Tablet.

Drug-Disease Interaction: If you have beriberi disease caused by vitamin B1 deficiency, malabsorption syndrome (unable to absorb minerals from food), thyroid, or any lung or kidney disease, please inform your doctor before taking Agoxin 0.25mg Tablet.

✔️ Contraindications

If you are known to be allergic to Agoxin 0.25mg Tablet or any other medicines, please tell your doctor.

✔️ Pregnancy & Lactation

Pregnancy:

  • Risk: Use with caution. Although no significant risk of birth defects has been identified, underlying maternal conditions like heart failure and atrial fibrillation can pose risks to both mother and fetus. Digoxin crosses the placenta and may require dose adjustments during pregnancy and the postpartum period.

Lactation:

  • Risk: Digoxin does enter breast milk, but the dose received by the infant is generally low and not clinically significant. However, monitoring is still recommended. The American Academy of Pediatrics considers digoxin "compatible with nursing."
✔️ Considerations for Use of Digoxin:

Cardiac Dysrhythmias:

  • Risk: Digoxin can exacerbate certain types of arrhythmias. Close monitoring is necessary, especially in patients with preexisting dysrhythmias.

Electrolyte Imbalances:

  • Hypokalaemia, Hypercalcaemia, Hypomagnesaemia: Electrolyte imbalances can increase the risk of digoxin toxicity. These conditions should be corrected before starting digoxin therapy.

Hypertension and Ischemic Heart Disease (IHD):

  • Risk: Use with caution as these conditions may alter the hemodynamic response to digoxin.

Other Cardiac Conditions:

  • Chronic Cor Pulmonale, Aortic Valve Disease, Acute Myocarditis, Congestive Cardiomyopathies, Constrictive Pericarditis: Patients with these conditions require careful monitoring, as digoxin may worsen symptoms or lead to complications.
  • Heart Block: Digoxin can worsen heart block, particularly in patients with pre-existing conditions. It should be used with extreme caution.

Elderly Patients:

  • Risk: Due to reduced renal function and increased sensitivity, lower doses are typically required to avoid toxicity.

Renal Impairment:

  • Risk: Digoxin is primarily excreted by the kidneys, so dosage must be adjusted to avoid accumulation and toxicity in patients with renal impairment.

Thyroid Function Abnormalities:

  • Risk: Both hyperthyroidism and hypothyroidism can affect digoxin metabolism and its effects. Monitoring and dosage adjustments are required.
✔️ Storage:

Store in a cool and dry place. Keep out of the reach of children.
