✔️ Indications

Entresto 100 is primarily used for the treatment of chronic heart failure.

✔️ Side Effects (By Frequency)

• Low blood pressure (18%)
• High potassium levels (12%)

• Orthostatic hypotension (2.1%)

✔️ Entresto 100 is an angiotensin receptor-neprilysin inhibitor (ARNI):

• Sacubitril inhibits neprilysin, which normally breaks down beneficial heart-protective peptides. This leads to increased urine output, lower blood pressure, and reduced heart strain.
• Valsartan blocks the angiotensin II type 1 receptor, relaxing blood vessels and reducing aldosterone effects, which helps lower blood pressure and prevent fluid retention.

✔️ Dosage & Administration

Adults (Heart Failure Treatment)

• Starting dose: 49 mg/51 mg taken twice daily.
• Target dose: Increase to 97 mg/103 mg twice daily after 2-4 weeks, as tolerated.

For Liver Impairment:

• Mild impairment (Child-Pugh A): No dose adjustment needed.
• Moderate impairment (Child-Pugh B): Start with 24 mg/26 mg twice daily and gradually increase.
• Severe impairment (Child-Pugh C): Not recommended.

For Kidney Impairment:

• Mild-to-moderate (eGFR ≥30 mL/min/1.73m²): No dose adjustment needed.
• Severe (eGFR <30 mL/min/1.73m²): Start with 24 mg/26 mg twice daily, increasing gradually.

✔️ Quick Suggestions:

• Stay active with light exercises such as walking and stretching.
• Follow a low-sodium diet to manage heart failure effectively.
• Rise slowly from sitting or lying down to prevent dizziness.
• Avoid potassium supplements and potassium-rich foods (e.g., bananas, broccoli) as Entresto may raise potassium levels.
• Inform your doctor if you have liver or kidney disease.
• Not safe during pregnancy or breastfeeding—avoid use if you are pregnant or nursing.
• Regular blood pressure and kidney function monitoring is required while on this medication.

✔️ Contraindications (Who Should NOT Take Entresto 100)

• Allergic reactions to sacubitril, valsartan, or any component of the tablet.
• History of angioedema due to previous ACE inhibitors or ARBs.
• Concurrent use of ACE inhibitors—avoid taking within 36 hours of switching.
• Use with aliskiren in diabetic patients.

• Risk of birth defects—do not use during pregnancy.
• May cause angioedema—seek medical help if swelling of the face, lips, or throat occurs.
• May cause low blood pressure—higher risk in dehydrated or salt-depleted patients.
