Description:

Bromocriptine belongs to a class of medications known as ergot derivatives and is used to treat Parkinson's disease, a neurological disorder. It can also be used to treat infertility problems (such as delay in puberty or lack of periods in both men and women) and prolactinomas (non-cancerous tumors of the brain) which are caused due to increased levels of prolactin in the blood.

Pharmacology

Bromocriptine is a dopamine receptor agonist that primarily inhibits the secretion of prolactin by activating post-synaptic dopamine receptors. In acromegaly, it lowers elevated blood levels of growth hormone. It significantly reduces plasma levels of prolactin in patients with both physiologically elevated prolactin and hyperprolactinemia.

Dosage & Administration of Bromodel 2.5mg Tablet

Adult Dosage:

Parkinson's Disease (as an adjunct to levodopa):

  • 1st week: 1-1.25 mg at night
  • 2nd week: 2-2.5 mg at night
  • 3rd week: 2.5 mg twice daily
  • 4th week: 2.5 mg three times daily, then increase as needed.
  • Maintenance: 10-40 mg/day

Hyperprolactinemia:

  • Initial: 1.25-2.5 mg daily
  • May increase by 2.5 mg/day every 2-7 days
  • Usual therapeutic dosage: 5-7.5 mg/day, ranges from 2.5-15 mg/day
  • Up to 30 mg/day has been used in some patients with amenorrhea and/or galactorrhea

Acromegaly (Parlodel):

  • Initial: 1.25-2.5 mg at bedtime for 3 days
  • May increase by 1.25-2.5 mg/day every 3-7 days
  • Not to exceed 100 mg/day
  • Adjunct to surgery and radiotherapy: Initial 2.5 mg twice daily, increase slowly up to 5 mg every 6 hours if needed

Lactation Suppression:

  • 2.5 mg/day for 2-3 days, then up to 2.5 mg twice daily for 14 days

Prevention of Puerperal Lactation:

  • 2.5 mg on the day of delivery, then 2.5 mg twice daily for 14 days

Cyclical Benign Breast and Menstrual Disorders:

  • 2.5 mg twice daily

Hepatic Impairment:

  • Dose adjustment may be necessary; there are no specific guidelines

Child Dosage:

  • Hyperprolactinemia (Ages 11-15 years):
    • Initial: 1.25-2.5 mg daily
    • Maintenance: 2.5-10 mg/day as advised by your physician
Administration Instructions:

Pregnancy 

  • The safety of Bromocriptine treatment during pregnancy for both the mother and fetus has not been fully established.
  • If pregnancy occurs during Bromocriptine administration, careful observation of these patients is mandatory.
  • Prolactin-secreting adenomas may expand, causing compression of the optic or other cranial nerves, which may necessitate emergency pituitary surgery; in most cases, this compression resolves after delivery.
  • Reinitiation of Bromocriptine treatment has been reported to improve visual fields in patients who experienced nerve compression during pregnancy.
  • For patients being treated for hyperprolactinemia, Bromocriptine should be withdrawn when pregnancy is diagnosed.
  • In patients with acromegaly, prolactinoma, or Parkinson's disease who become pregnant, it should be determined whether continuing therapy is medically necessary or if it can be discontinued.
  • If therapy is continued, it should be discontinued in those who may experience hypertensive disorders of pregnancy (e.g., eclampsia, preeclampsia, or pregnancy-induced hypertension) unless Bromocriptine withdrawal is considered medically contraindicated.

Lactation:

  • Avoid use during lactation in postpartum women.
