✔️ How does Synthox 50 work?

Synthox-50 is a synthetic version of a hormone produced by the thyroid gland. It works by replacing the thyroid hormones that your thyroid gland cannot produce in a sufficient quantity, and relieves the symptoms of hypothyroidism (tiredness, weight gain, and depression).

✔️ Dosage of Synthox 50

Adult Dosage:

• Initial dose: 25–50 mcg/day, with dose adjustments (12.5–25 mcg increments) every 6–8 weeks until serum TSH normalizes and the patient is euthyroid.
• Severe hypothyroidism: Start with 12.5–25 mcg/day, increasing by 25 mcg every 2–4 weeks until TSH normalizes.
• Secondary or tertiary hypothyroidism: Adjust the dose to achieve a euthyroid state and maintain free T4 levels in the upper normal range.
• Older adults or patients with cardiac conditions: Start at 1.7 mcg/kg/day.

Pediatric Dosage:

• Newborns: Start with 10–15 mcg/kg/day. Lower doses may be needed for infants at risk of heart failure, with adjustments every 4–6 weeks.
• For newborns with very low T4 (<5 mcg/dL), begin with 50 mcg/day.
• Infants and children: Start with 25 mcg/day, increasing by 25 mcg every 2–4 weeks. Adjust according to clinical response.
  • Age-based dosing:
    • 0–3 months: 10–15 mcg/kg/day
    • 3–6 months: 8–10 mcg/kg/day
    • 6–12 months: 6–8 mcg/kg/day
    • 1–5 years: 5–6 mcg/kg/day
    • 6–12 years: 4–5 mcg/kg/day

12 years (growth and puberty incomplete): 2–3 mcg/kg/day

• After growth and puberty: 1.7 mcg/kg/day

✔️ Administration: Synthox 50

If you miss a dose of Synthox 50, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose. Your doctor will decide the correct dose and duration for you depending on your age, body weight, and disease condition. Do not stop taking Synthox 50 unless your doctor advises.

✔️ Adverse effects are typically due to excessive dosing, leading to symptoms of hyperthyroidism, such as:

• General: Fatigue, weight loss, heat intolerance, excessive sweating.
• Neurological: Headache, nervousness, irritability, insomnia.
• Musculoskeletal: Muscle weakness, tremors.
• Cardiovascular: Palpitations, tachycardia, arrhythmias, increased blood pressure.
• Respiratory: Shortness of breath.
• Gastrointestinal: Diarrhea, vomiting, abdominal cramps.
• Dermatological: Hair loss, skin flushing.

✔️ Symptoms of overdose mimic hyperthyroidism and may include:

• Agitation, irritability, hyperactivity, headache, sweating, rapid heart rate, and convulsions.
• Severe cases: Cerebral embolism, shock, coma, and even death.
• Management: Reduce or temporarily discontinue Synthox-50, with symptomatic treatment as needed.

✔️ Drug Interactions:

• Tricyclic or tetracyclic antidepressants: Concurrent use may amplify therapeutic and toxic effects, including cardiac arrhythmias and CNS stimulation.
• Sertraline: May increase Synthox-50 requirements.
• Diabetes medications/Insulin: Starting or adjusting Synthox-50 may require changes in diabetes treatment. Monitor blood glucose levels.
• Digitalis glycosides: Reduced therapeutic effects may occur when transitioning from hypothyroid to euthyroid states.

✔️ Synthox-50 is contraindicated in:

• Untreated subclinical or overt thyrotoxicosis.
• Acute myocardial infarction.
• Uncorrected adrenal insufficiency.

✔️ Pregnancy & Lactation:

• Pregnancy: Category A. Levothyroxine requirements may increase during pregnancy. Adequate replacement therapy is generally safe and needed to maintain normal lactation.
• Lactation: Although thyroid hormones are minimally excreted in breast milk, caution is advised.

✔️ Precautions & Warnings:

• Adrenal insufficiency: Address with corticosteroids before starting Synthox-50 to avoid adrenal crisis.
• Cardiovascular risks: Use with caution in older adults or those with heart disease, as rapid dose increases may worsen angina, arrhythmias, or myocardial infarction.
• Adjust the dose for significant weight changes during treatment.
• Monitor serum TSH levels regularly to determine appropriate dosing and prevent overdose.
