How does Cardobis M 2.5/5mg Tablet work?

• Bisoprolol (beta blocker) helps relax our blood vessels by blocking the action of certain natural substances in your body. This lowers your blood pressure and helps in reducing your risk of having a stroke, a heart attack, other heart problems, or kidney problems in the future. This medicine needs to be taken regularly to be effective.
• Amlodipine is a calcium channel blocker that acts by relaxing the blood vessels. This reduces the workload on the heart and makes the heart more efficient at pumping blood throughout the body.

Side Effects of Cardobis M 2.5/5mg Tablet

Common:

• Dizziness, headache, somnolence
• Palpitations, flushing
• Gastrointestinal complaints (nausea, vomiting, diarrhea, constipation, abdominal pain)
• Edema (e.g., ankle edema), fatigue

Uncommon:

• Insomnia, mood changes, depression
• Visual disturbances, tinnitus
• AV conduction disturbances, worsening heart failure
• Dyspnea, bronchospasm, rhinitis
• Alopecia, purpura, pruritus, exanthema
• Arthralgia, myalgia, muscular weakness, cramps
• Micturition disorder, nocturia, pollakiuria
• Potency disorders, gynecomastia
• Asthenia, chest pain, weight changes

Rare:

• Allergic reactions, nightmares, hallucinations
• Hepatitis, increased liver enzymes

• Do regular physical activity or exercise for at least 150 minutes per week, or about 30 minutes most days of the week. Doing this can help you to lower your raised blood pressure by about 5 mm of Hg.
• If you are taking alcohol then only one serving for women and two servings for men is advisable.

Indication of Cardobis M 2.5/5mg Tablet

Cardobis M 2.5/5mg Tablet is used to treat high blood pressure. It works by relaxing the blood vessels. This reduces the workload on the heart and makes the heart more efficient at pumping blood throughout the body.

This combination consists of Amlodipine and Bisoprolol Fumarate:

• Amlodipine: A dihydropyridine calcium antagonist that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, reducing peripheral vascular resistance and blood pressure.
• Bisoprolol Fumarate: A beta1-selective adrenoceptor blocking agent with no intrinsic sympathomimetic activity, reducing the heart rate and myocardial contractility, thus lowering blood pressure.

Dosage & Administration of Cardobis M 2.5/5mg Tablet

• One tablet once daily in patients whose blood pressure is adequately controlled with separately administered monocomponent products of the same doses as the recommended fixed-dose combination.
• Elderly: Usual doses can be administered, but caution is advised when increasing the dose.
• Pediatric: Safety and efficacy not established for children and adolescents under 18 years.
• Liver Disease: Use with caution; severe impairment requires careful dosing.
• Kidney Disease: No adjustment is needed for mild/moderate impairment; caution in severe impairment.

Interaction

Not recommended:

• Calcium antagonists (verapamil, diltiazem)
• Centrally-acting antihypertensive drugs

Use with caution:

• Strong/moderate CYP3A4 inhibitors and inducers
• Simvastatin, Tacrolimus, Cyclosporine
• Antiarrhythmic drugs (class I and III)
• Parasympathomimetic drugs
• Topical beta-blockers (e.g., eye drops)
• Insulin and oral antidiabetic drugs
• Anesthetic agents, digitalis glycosides
• NSAIDs, sympathomimetic agents
• Other antihypertensive agents

Consider:

• Mefloquine, Rifampicin
• Ergotamine derivatives
• MAO inhibitors (except MAO-B inhibitors)

Contraindications

• Acute heart failure
• Episodes of heart failure decompensation
• Obstruction of the outflow tract of the left ventricle (e.g., high-grade aortic stenosis)
• Cardiogenic shock
• Second or third-degree AV block, sick sinus syndrome, sinoatrial block
• Symptomatic bradycardia or hypotension
• Severe bronchial asthma
• Severe peripheral arterial occlusive disease or Raynaud’s syndrome
• Untreated pheochromocytoma
• Metabolic acidosis
• Hypersensitivity to bisoprolol, amlodipine, dihydropyridine derivatives, or any excipients

Pregnancy & Lactation

Not recommended during pregnancy and lactation.

Precautions & Warnings

• Caution in patients with heart failure, hepatic impairment, obstructive airway disease, diabetes, and strict fasting.
• Beta-blockers should be used with caution in patients with bronchospasm, diabetes with large fluctuations in blood glucose, undergoing desensitization therapy, first-degree AV block, Prinzmetal’s angina, and peripheral arterial occlusive disease.
• Patients with psoriasis should be given beta-blockers after careful consideration.
• Symptoms of thyrotoxicosis may be masked.
• Abrupt withdrawal may lead to clinical deterioration; gradual reduction is recommended.
