Indication of Nebivolol Tablet

Nebipro 2.5 belongs to the group of medicines called 'beta-blockers' used to treat hypertension (high blood pressure). Nebipro 2.5 is also used to treat mild to moderate heart failure in patients aged 70 years and older in addition to other therapies. 

Pharmacology of Nebivolol:

Nebivolol is a β-adrenergic receptor blocker that acts selectively on both β1 and β2 adrenergic receptors. The drug has no intrinsic sympathomimetic or membrane-stabilizing activity at therapeutic concentrations. Its beta-blocking activity is primarily due to its d-nebivolol isomer and metabolites like glucuronides, which contribute to the overall effect.

Mode of Action:

  1. Decreased heart rate: By inhibiting β1 receptors, Nebivolol reduces the heart's rhythm and overall workload.
  2. Decreased myocardial contractility: Lowers the force of contraction, reducing oxygen demand on the heart.
  3. Sympathetic outflow suppression: Nebivolol diminishes tonic sympathetic signals from the brain to peripheral blood vessels.
  4. Renin suppression: Lowers renin activity in the kidneys, contributing to decreased blood pressure.
  5. Vasodilation: Induces widening of blood vessels, reducing peripheral vascular resistance.

Pharmacokinetics:

  • Metabolism: Nebivolol undergoes glucuronidation and hydroxylation primarily via the enzyme CYP2D6. The active d-nebivolol isomer has a half-life of approximately 12 hours in extensive metabolizers (EMs) and 19 hours in poor metabolizers (PMs).
  • Absorption: After oral administration, peak plasma levels are reached within 1.5 to 4 hours. Food does not significantly affect absorption.
  • Protein Binding: Nebivolol is 98% protein-bound in plasma, predominantly to albumin, irrespective of its concentration.
  • Excretion: EMs eliminate nebivolol primarily through feces (44%) and urine (38%), while PMs excrete more in urine (67%) than feces (13%).
Dosage & Administration: Nebipro 2.5mg Tablet

For Hypertension (High Blood Pressure):

  • Adults: Initially, 5 mg once daily. If needed, the dose can be increased at 2-week intervals.
    • Maximum dose: 40 mg once daily.
  • Elderly (65 years and older): Initially, 2.5 mg once daily, which may be increased to 5 mg once daily if required.

For Heart Failure:

  • Adults: Initially, 1.25 mg once daily. If tolerated, the dose may be doubled every 1-2 weeks.
    • Maximum dose: 10 mg once daily.

For Hepatic Impairment:

  • Start with 2.5 mg once daily, and increase cautiously based on tolerance.

For Renal Impairment:

  • CrCl <30 mL/min: Start with 2.5 mg once daily, and increase to 5 mg once daily if needed.

Dosage should always be adjusted by a healthcare provider based on individual needs and response to treatment.

Swallow Nebipro 2.5mg Tablet with a glass of water as a whole. Do not chew, break, or crush it. You can take Nebipro 2.5mg Tablet with or without food. Take Nebipro 2.5mg Tablet at the same time daily for the best results. Based on your medical condition, your doctor will advise for how long you need to take Nebipro 2.5mg Tablet.

Drug Interactions:

Drug-Drug Interactions: Nebivolol may interact with:

  • Painkillers: Aspirin, ibuprofen.
  • Diuretics: Chlorthalidone, furosemide, hydrochlorothiazide.
