Nifecap is indicated for managing all types of essential and renal hypertension. It is also used to manage hypertension during pregnancy and coronary bypass surgery. Additionally, Nifecap is used for the prevention and treatment of unstable and variant angina, myocardial infarction, silent myocardial ischemia, Raynaud's phenomenon, and heart failure.
Nifedipine, a calcium channel blocker, inhibits the transmembrane influx of calcium ions into muscle cells. It selectively dilates arterial vessels, including major coronary and systemic arteries, leading to a reduction in blood pressure. This decrease in blood pressure triggers a sympathetic reflex response, causing tachycardia and increased cardiac output. Pulmonary arterial pressure is also reduced. At higher doses, Nifedipine has direct negative inotropic effects on cardiac muscles, which are less pronounced than its vasodilatory effects.
For patients not previously prescribed Nifedipine, start with one tablet once daily. For hypertension and angina prophylaxis, the recommended dose is 20 mg twice daily with food. The dosage may be adjusted from 10 mg twice daily to 40 mg twice daily.
Nifecap is contraindicated in patients with cardiogenic shock, advanced aortic stenosis, nursing mothers, gastrointestinal obstruction, inflammatory bowel disease, and hypotension.
There are no adequate and well-controlled studies in pregnant women. Nifecap should be used during pregnancy only if the potential benefits justify the potential risks to the fetus.
