Clarithromycin works by inhibiting protein synthesis in susceptible bacteria. It binds to the donor site on the 50S subunit of the bacterial ribosome, blocking translocation. This antibiotic is effective against most Gram-positive bacteria, Chlamydia, some Gram-negative bacteria, and Mycoplasmas. Clarithromycin demonstrates equal or greater activity compared to Erythromycin against most Gram-positive bacteria and is more acid-stable, enhancing tolerance. It exhibits twice the activity of Erythromycin against Haemophilus influenzae, while most Gram-negative bacteria are resistant due to penetration issues.
Adults:
- Acute bacterial exacerbation of chronic bronchitis: 250-500 mg every 12 hours for 7-14 days.
- Acute maxillary sinusitis: 500 mg every 12 hours for 14 days.
- Community-acquired pneumonia: 250 mg every 12 hours for 7-14 days.
- Pharyngitis or tonsillitis: 250 mg every 12 hours for 10 days.
- Uncomplicated skin infections: 250 mg every 12 hours for 7-14 days.
- Mycobacterium avium disease: 500 mg every 12 hours until low risk for infection.
H. pylori Infection (Combination Therapy):
- Triple Therapy: 500 mg clarithromycin, 30 mg lansoprazole, and 1 g amoxicillin every 12 hours for 10-14 days.
- Triple Therapy: 500 mg clarithromycin, 20 mg omeprazole, and 1 g amoxicillin every 12 hours for 10-14 days, with an additional 14-18 days of omeprazole for ulcer healing if necessary.
- Dual Therapy: 500 mg clarithromycin every 8 hours and 40 mg omeprazole once daily for 14 days, with an additional 14 days of omeprazole 20 mg for ulcer healing.
Children:
- 15 mg/kg/day divided every 12 hours for 10 days.
- For mycobacterial infections: 7.5 mg/kg every 12 hours, up to 500 mg.
Renal and Hepatic Impairment:
Clarithromycin can be given without dosage adjustment in hepatic impairment with normal renal function. Adjust dosage or prolong intervals in severe renal impairment.
