Oral (Adults):
500 mg once daily for 3 days OR
500 mg on day 1, then 250 mg daily for 2–4 days
For STIs (Chlamydia):
1 g single dose OR 500 mg day 1 + 250 mg daily for 2 days
Oral (Children):
6 months: 10 mg/kg once daily for 3 days
15–25 kg: 200 mg daily
26–35 kg: 300 mg daily
36–45 kg: 400 mg daily
Typhoid fever: 500 mg once daily for 7–10 days
IV Injection (Adults):
Community-acquired pneumonia: 500 mg IV once daily for ≥2 days, then switch to oral 500 mg daily to complete 7–10 days
Pelvic inflammatory disease: 500 mg IV once daily for 1–2 days, then switch to oral 250 mg daily for 7 days
Use in children <16 years for IV therapy is not established
Oral Administration Notes:
Take at least 1 hour before or 2 hours after meals
Suspension Reconstitution:
Shake bottle to loosen powder
Add boiled and cooled water to the mark
Shake until fully dissolved
Antacids: Take at least 1 hour before or 2 hours after
Cyclosporin: Monitor levels due to potential interaction
Digoxin: Monitor levels; may impair metabolism
Ergot derivatives: Avoid co-administration
Warfarin: Can be co-administered; monitor prothrombin time
Other drugs (carbamazepine, methylprednisolone, terfenadine, theophylline, terfenadine): Generally safe, but monitor as needed
Hypersensitivity to azithromycin or other macrolides
Co-administration with ergot derivatives
Patients with hepatic disease
Common: Nausea, vomiting, abdominal discomfort, diarrhea, flatulence, loose stools
Allergic reactions: Rash, photosensitivity
Rare: Serious hypersensitivity, reversible liver enzyme elevation, cholestatic jaundice, mild transient neutrophil reduction, reversible hearing impairment at high doses
Category B: No evidence of fetal harm in animal studies; use only if necessary in humans
Unknown if excreted in breast milk; exercise caution
Rare serious allergic reactions including angioneurotic edema and anaphylaxis
Avoid in hepatic impairment
Use caution in renal impairment (creatinine clearance <40 mL/min)
