How does Oleanz 5mg Tablet work?

Olanzapine is an atypical antipsychotic. It works by affecting the levels of chemical messengers (dopamine and serotonin) to improve mood, thoughts, and behavior.

Side Effects of Oleanz 5mg Tablet

• Common: Somnolence, weight gain, increased appetite.
• Others: Elevated glucose/triglyceride levels, dizziness, akathisia, Parkinsonism, dyskinesia, orthostatic hypotension, anticholinergic effects (constipation, dry mouth), asthenia, edema, photosensitivity reactions.

• Olanzapine works well in schizophrenia and bipolar disorder.
• It is less likely to cause heart and movement problems as compared to other similar medicines.
• It may take 4 to 6 weeks to see full effects on thoughts and mood. Keep taking it as prescribed.
• Use caution while driving or doing anything that requires concentration as Olanzapine can cause dizziness and sleepiness.
• It may increase your weight, blood sugar, and cholesterol. Eat healthy, exercise regularly, and monitor your blood levels regularly.
• To lower the chance of feeling dizzy or passing out, rise slowly if you have been sitting or lying down.
• Do not stop taking Olanzapine without talking to your doctor first as it may cause worsening of symptoms.

Indications:

• Schizophrenia: Treats both positive (delusions, hallucinations, disordered thinking, anger, suspiciousness) and negative symptoms (flattened affect, emotional/social withdrawal, speech poverty).
• Bipolar Disorder: Used for acute manic or mixed episodes, with or without psychotic symptoms, and rapid cycling.

Pharmacology

• Mechanism of Action: Binds to serotonin (5HT2A/2C, 5HT3, 5HT6), dopamine (D1-D5), cholinergic muscarinic (M1-M5), adrenergic (α1), and histamine (H1) receptors. Effectiveness is likely due to dopamine and serotonin type 2 (5HT2) antagonism.
• Absorption: Rapid absorption after oral intake; peak plasma concentrations in 5-8 hours. Food does not affect absorption.
• Safety: Not carcinogenic, mutagenic, or clastogenic.

Adult Dose of Oleanz 5mg Tablet

Schizophrenia

• Initial Dose: 5-10 mg/day
• Titration: Increase in increments of 5 mg/day at intervals >1 week
• Maintenance: 10-20 mg/day
• Maximum Dose: 20 mg/day

Bipolar Mania

• Monotherapy: 10-15 mg/day initially
• Adjunct to Lithium or Valproate: 10 mg/day initially
• Maintenance: 5-20 mg/day
• Maximum Dose: 20 mg/day

Depression in Bipolar Disorder

• Combination with Fluoxetine: Start with 5 mg in the evening
• Adjustment: Range of 5-12.5 mg/day
• Resistant Depression: May increase up to 20 mg/day

• Renal Impairment
  • Initial Dose: 5 mg/day

Interactions

• Dopamine Agonists/Levodopa: Olanzapine may counteract the effects.
• CYP1A2 or Glucuronyl Transferase Inducers: E.g., omeprazole, and rifampicin may enhance olanzapine elimination.
• CYP1A2 Inhibitors: May reduce clearance.
• Carbamazepine: May increase clearance.
• Activated Charcoal: Reduces oral bioavailability by 50-60%.
• Caution: When used with centrally acting medications and alcohol.

Contraindications

• Known hypersensitivity to olanzapine or its ingredients.
• Risk for narrow-angle glaucoma.

Pregnancy & Lactation

• Pregnancy: Use only if the benefits outweigh the risks to the fetus. No evidence of teratogenicity.
