How does Nalbun Injection work?

Nalbun blocks pain signals, so the brain doesn't perceive the severity of pain, hence increasing the body's threshold for pain. 

Side Effects of Nalbun Injection

Commonly well tolerated but may include:

• Sedation
• Sweating
• Nausea
• Vomiting
• Dizziness
• Vertigo
• Dry mouth
• Headache
• Respiratory depression
• Dyspnea
• Asthma

Quick Suggestions:

• Use Nalbuphine as instructed by your doctor. Never take in greater amounts or more often than prescribed.
• It causes sleepiness or drowsiness. If this happens to you, do not drive or use machinery.
• Avoid consuming alcohol when taking the Nalbuphine, as it may cause excessive sleepiness or drowsiness.
• Inform your doctor if you have ever been diagnosed with kidney or liver problems.
• Store opioid pain relievers in a safe place and out of reach of others.

Indication of Nalbun Injection

Nalbuphine is used for moderate to severe pain.

Pharmacology

Nalbuphine Hydrochloride is a synthetic, potent analgesic with effects comparable to morphine. It functions primarily as a kappa agonist and partial mu antagonist. Key pharmacological properties include:

• Receptor Binding: Binds to mu, kappa, and delta receptors, but not sigma receptors.
• Onset of Action: 2 to 3 minutes after intravenous administration; less than 15 minutes after subcutaneous or intramuscular injection.
• Plasma Half-Life: Approximately 5 hours.
• Duration of Analgesic Activity: Ranges from 3 to 6 hours.

Dosage & Administration of Nalbun Injection

Adults:

Moderate to Severe Pain:

• 10-20 mg for a 70 kg patient, via intramuscular or intravenous injection.
• Adjust dosage based on pain severity, patient’s physical status, and other medications.
• Repeat every 3 to 6 hours as necessary.

Preoperative Anesthesia:

• 0.1-0.2 mg/kg via intravenous or intramuscular injection.

Obstetrical Analgesia (Labor & Delivery):

• 0.3-1 mg/kg via intravenous injection over 10-15 minutes.
• Maintenance doses of 0.25-0.5 mg/kg as needed.

Intraoperative Analgesia:

• 0.25-0.5 mg/kg via intravenous injection at 30-minute intervals.

Myocardial Infarction:

• 10-20 mg via slow intravenous injection.
• Repeat after 30 minutes if necessary.

Children (18 months to 15 years old):

• Analgesia:
  • 0.2 mg/kg body weight, preferably via intravenous or intramuscular injection.
  • Maintenance doses every 4 to 6 hours, as determined by a physician.

Interaction

• No hazardous interactions were identified specifically for Nalbun.
• Anticipated interactions include additive effects when combined with:
  • Narcotic analgesics
  • General anesthesia
  • Phenothiazines
  • Tranquilizers
  • Sedatives
  • Hypnotics
  • CNS depressants (including alcohol)

Contraindications

Known hypersensitivity to Nalbuphine Hydrochloride.

Pregnancy & Lactation

• Pregnancy Category B: High and rapid placental transfer. Use only if clearly needed.
• Lactation: Limited data suggest minimal excretion in maternal milk with no significant clinical effect. Exercise caution when administering to nursing women.

Precautions & Warnings

Use with caution in:

• Impaired respiration
• Impaired renal or hepatic function
• Biliary tract surgery
• Myocardial infarction
• Hypotension
