How does Nicorandil work?

Nicorandil works by dilating arterioles and large coronary arteries through the opening of potassium channels and stimulating guanylate cyclase, leading to venous vasodilation. This action reduces preload and afterload, enhancing coronary blood flow.

Side Effects of Nicorandil

Common side effects include headache (usually temporary), flushing, dizziness, nausea, vomiting, and weakness. High doses can cause hypotension and reflex tachycardia.

Pharmacology

Nicorandil, the active ingredient in Nictoris, works by dilating arterioles and large coronary arteries through the opening of potassium channels. It also stimulates guanylate cyclase, leading to venous vasodilation. This action reduces both preload and afterload, improving coronary blood flow.

Dosage & Administration of Nicorandil

• Adults: The standard dose range is 10 to 20 mg twice daily. The starting dose is typically 10 mg twice daily, taken in the morning and evening, and may be increased based on the patient's needs, response, and tolerance, up to 40 mg twice daily. For patients prone to headaches, a lower starting dose of 5 mg twice daily may be used.
• Elderly: Use the lowest effective dose, administered with caution.
• Children: Not recommended.
• Hepatic Impairment: Dose adjustment may be required.

Interaction

While studies have shown no significant interactions with beta-blockers, calcium antagonists, digoxin, a digoxin/furosemide combination, rifampicin, or cimetidine, Nicorandil might still enhance the effects of other vasodilators, tricyclic antidepressants, and antihypertensive drugs, especially when combined with alcohol.

Contraindications

• Hypersensitivity to Nicorandil

• Acute myocardial infarction with acute left ventricular failure and low filling pressures
• Hypotension
• Concurrent use with phosphodiesterase-5 inhibitors, due to the risk of severe hypotension

Pregnancy & Lactation

• Pregnancy: Animal studies show no harm to the fetus, but human studies are lacking. Use only if the potential benefits outweigh the risks.
• Lactation: Unknown if Nicorandil is excreted in human milk. Use caution when administering to nursing mothers.

Precautions & Warnings

• Use cautiously in patients with blood volume depletion or low systolic blood pressure (below 100 mm Hg).
• Avoid use in patients with cardiogenic shock or acute myocardial infarction with acute left ventricular failure and low filling pressures.
• Discontinue use if mouth ulceration, stomatitis, or severe buccal ulcerations occur.
• Use with caution in patients with glaucoma.
• Nicorandil can enhance the hypotensive effects of other vasodilators, tricyclic antidepressants, and alcohol.
• In hypertensive patients, monitor blood pressure closely when combined with antihypertensive medications.
