Nicorandil works by dilating arterioles and large coronary arteries through the opening of potassium channels and stimulating guanylate cyclase, leading to venous vasodilation. This action reduces preload and afterload, enhancing coronary blood flow.
Common side effects include headache (usually temporary), flushing, dizziness, nausea, vomiting, and weakness. High doses can cause hypotension and reflex tachycardia.
