Xenole 375mg Tablet is a Non-Steroidal Anti-Inflammatory Drug (NSAID) used to reduce and relieve pain. It combines Naproxen, a painkiller, and esomeprazole, an antacid. Naproxen works by blocking the COX enzyme to reduce pain and inflammation, while esomeprazole reduces stomach acid to prevent gastric discomfort and ulcers caused by Naproxen.

Xenole 375mg Tablet is a combination of two medicines: Naproxen and Esomeprazole which relieve pain.

Pharmacology

Xenole 375mg Tablet consists of an immediate-release esomeprazole magnesium layer and an enteric-coated naproxen core. Esomeprazole is released first into the stomach, followed by naproxen in the small intestine.

Naproxen is an NSAID with analgesic and antipyretic properties that inhibits prostaglandin synthesis. Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by inhibiting the H+/K+-ATPase in gastric parietal cells, thereby reducing gastric acidity.

Dosage & Administration of Xenole 375mg Tablet

Carefully weigh the benefits and risks before deciding to use this medication. Use the lowest effective dose for the shortest duration consistent with individual treatment goals. If a total daily dose of esomeprazole lower than 40 mg is needed, consider a different treatment.

  • Rheumatoid Arthritis, Osteoarthritis, and Ankylosing Spondylitis (Adults): One tablet twice daily (either 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole).
  • Juvenile Idiopathic Arthritis (Adolescents 12 years and older):
    • Weight >50 kg: One tablet twice daily (either 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole).
    • Weight 38-50 kg: One tablet twice daily of 375 mg naproxen/20 mg esomeprazole.

Administration

Do not split, chew, crush, or dissolve the tablet. Take at least 30 minutes before meals.

Interaction

With other medications:

  • NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers.
  • Concomitant use of warfarin may increase the risk of bleeding complications.
  • Esomeprazole may interfere with the absorption of drugs where gastric pH is important for bioavailability (e.g., ketoconazole, iron salts, digoxin).

With food:

  • High-fat food does not affect naproxen absorption but prolongs tmax by 10 hours and decreases Cmax by about 12%.
Contraindications
  • Known hypersensitivity to any component of this medication or substituted benzimidazoles.
  • History of asthma, urticaria, or other allergic reactions after taking aspirin or other NSAIDs.
  • Use during the peri-operative period for coronary artery bypass graft (CABG) surgery.
Pregnancy & Lactation
  • Pregnancy: Avoid unless the benefits outweigh the risks, especially during the first and second trimesters. Contraindicated during the third trimester.
  • Breastfeeding: Naproxen is excreted in low quantities in breast milk. It is unknown if esomeprazole is excreted. Avoid use during breastfeeding.
  • Fertility: NSAIDs like naproxen may impair female fertility. Not recommended for women attempting to conceive.
Precautions & Warnings
  • General: Avoid combining with other NSAIDs, including selective cyclooxygenase-2 inhibitors, due to cumulative risks. Can be used with low-dose aspirin.
  • Elderly: Higher risk of adverse reactions, particularly gastrointestinal bleeding and perforation.
  • Gastrointestinal: Risk of GI bleeding, ulceration, or perforation increases with NSAIDs. Use with caution in patients with a history of GI disease.
  • Cardiovascular and Cerebrovascular: Monitor patients with hypertension, heart failure, or cardiovascular disease closely.
  • Renal: Long-term use can cause renal injury. Not recommended for moderate to severe renal impairment.
