✅ Description:
Risperidone, the active ingredient in Sperid, is a selective monoaminergic antagonist with a high affinity for serotonin (5-HT2) and dopamine (D2) receptors. It also binds to alpha-1 adrenergic receptors and, with lower affinity, to H1 histamine and alpha-2 adrenergic receptors. However, Risperidone does not interact with cholinergic receptors.
As a potent D2 antagonist, Risperidone helps reduce positive symptoms of schizophrenia, such as hallucinations and delusions. Its combined serotonin and dopamine antagonism may lower the risk of extrapyramidal side effects (movement disorders) and improve negative (e.g., social withdrawal) and affective (mood) symptoms of schizophrenia.
Dosage & Administration:
Psychoses:
- Day 1: 2 mg (1–2 divided doses)
- Day 2: 4 mg (1–2 divided doses)
- Usual Dose: 4–6 mg daily
- Maximum Dose: 16 mg daily (only if benefits outweigh risks)
- Elderly or Patients with Liver/Kidney Issues: Start at 1 mg daily (in two divided doses), increasing by 1–2 mg twice daily as needed.
- Children under 15 years: Not recommended.
Mania:
- Initial Dose: 2 mg once daily
- Increase by 1 mg daily if necessary.
- Usual Dose: 1–6 mg daily
- Elderly or Patients with Liver/Kidney Issues: Start at 1 mg daily (in two divided doses), increasing by 1–2 mg twice daily.
Schizophrenia:
- Initial Dose: 1 mg twice daily (BID)
- Increase by 1 mg BID on days 2 and 3.
- Further Adjustments: Adjust in 1–2 mg increments after at least 1 week if necessary.
Always use this medication according to your doctor’s advice.
Drug Interactions:
- Levodopa and dopamine agonists: Sperid may reduce their effectiveness.
- Carbamazepine: Chronic use reduces Sperid clearance from the plasma, potentially lowering its effectiveness.
- Clozapine: Chronic use may decrease Sperid clearance.
Contraindications:
Do not use Sperid if you are allergic to risperidone or any of its components.
Side Effects:
Common side effects may include:
- Insomnia
- Agitation
- Anxiety
- Headache
- Drowsiness (less common)
- Impaired concentration
- Fatigue
- Blurred vision
- Constipation
- Nausea and vomiting
- Indigestion (dyspepsia)
- Abdominal pain
- Increased prolactin levels (hyperprolactinemia)
- Urine incontinence
- Fast heart rate (tachycardia)
- High blood pressure (hypertension)
- Swelling (edema)
- Skin rash
- Runny nose (rhinitis)
- Stroke (cerebrovascular accident)
- Low white blood cell count (neutropenia)
- Low platelet count (thrombocytopenia)
Pregnancy & Lactation:
- Pregnancy: Animal studies show no evidence of birth defects (teratogenicity), but Risperidone’s safety in pregnant women is not established. Indirect effects related to prolactin and CNS may occur.
- Breastfeeding: Risperidone and its active metabolite, 9-hydroxyrisperidone, are excreted in breast milk. Women taking Sperid should avoid breastfeeding.
Precautions:
Use Sperid cautiously if you have:
- Preexisting cardiovascular disease
- Epilepsy
- Parkinson’s disease
- Kidney or liver impairment
- If signs of tardive dyskinesia (involuntary movements) appear, discontinue use immediately.
- Use cautiously during pregnancy.
Therapeutic Class:
Atypical neuroleptic (antipsychotic) drug.
