Ofloxacin: It is a fluoroquinolone antibiotic effective against a broad spectrum of bacteria commonly found in otic infections. Ofloxacin works by inhibiting the bacterial DNA gyrase, thereby preventing bacterial replication. It is active against both Gram-positive and Gram-negative bacteria, including Staphylococcus aureus, Streptococcus pneumoniae, Escherichia coli, Haemophilus influenzae, Moraxella catarrhalis, Proteus mirabilis, and Pseudomonas aeruginosa. Importantly, it does not cause ototoxicity, making it preferable over aminoglycosides in middle ear infections.
Clotrimazole: This synthetic imidazole derivative acts as a broad-spectrum antifungal agent, targeting fungi responsible for superficial mycotic infections affecting the outer or middle ear, such as Candida, Microsporum, and Trichophyton. Clotrimazole inhibits the synthesis of ergosterol, a crucial component of the fungal cell wall, leading to compromised cell wall integrity and inhibited fungal growth. Additionally, it exhibits antibacterial activity against certain bacteria like Streptococci and Staphylococci, making it effective against mixed bacterial-fungal infections.
Beclomethasone Dipropionate: As a potent corticosteroid, beclomethasone exerts anti-inflammatory, antipruritic, and vasoconstrictive effects. It works by inducing phospholipase A2 and subsequently inhibiting the release of arachidonic acid, leading to suppression of the formation, release, and activity of inflammatory mediators such as prostaglandins and leukotrienes. This helps alleviate inflammation and associated symptoms in the ear.
Lidocaine Hydrochloride: Lidocaine is a local anesthetic that acts by blocking pain signals from the nerves to the brain, thereby reducing pain and stinging sensations in the ear.
For Adults and Children: Instill 2-5 drops into the affected ear(s) three to four times daily.
For Otitis Externa with intact tympanic membrane: Suitable for individuals aged 6 months and older.
For Chronic Suppurative Otitis Media with perforated tympanic membrane: Recommended for individuals aged 12 years and older.
The treatment duration typically spans 7 to 14 days.
