Treatment of Melasma

How does Triquin Cream work?

Triquin Cream is a combination of three medicines: Hydroquinone, Tretinoin, and Fluocinolone acetonide which treat melasma (dark spots on the skin). Hydroquinone is a skin-lightening medicine. It reduces the amount of skin pigment (melanin) that causes the darkening of the skin. Tretinoin is a form of vitamin A that helps the skin to renew itself more quickly. Fluocinolone acetonide is a steroid that blocks the production of certain chemical messengers (prostaglandins) that make the skin red, swollen, and itchy.

Side Effects of Triquin Cream

• Possible severe allergic reactions include difficulty breathing or severe asthma attacks.
• Mild to moderate redness, peeling, burning, dryness, or itching of the skin may occur.

Quick Suggestions:

• Avoid sun exposure while using Triquin Cream as it may make the skin more sensitive to sunlight and cause sunburn. Wear protective clothing and use sunscreen while going out to protect your skin from sunburn.
• Regular exercise can improve your mood and self-esteem, though it doesn’t clear spots. Take a shower immediately after finishing the exercise.
• Wear a wide-brimmed hat to protect your face from sun exposure.
• Avoid using skin products that irritate, such as skin cleansers or shampoos, harsh soaps, hair removers or waxes, hair colors or permanent chemicals, and skin products with astringents, lime, spices, or alcohol.
• Avoid using Triquin Cream for more than 6-8 weeks without a doctor’s advice. Please consult a dermatologist if the skin condition does not improve after 2-3 months of treatment with Triquin Cream.
• Do not take Triquin Cream by mouth. Triquin Cream is for use only on the skin (topical use). If Triquin Cream gets in your eyes or mouth accidentally, immediately rinse it with cold water.

Indication

Triquin Cream is indicated for the short-term treatment of moderate to severe melasma of the face. It is important to avoid sun exposure after starting the treatment. Use sunscreen with at least SPF 30 during the day.

Pharmacology

This cream contains three active ingredients:

1. Fluocinolone Acetonide - A corticosteroid used as an anti-inflammatory agent.
2. Hydroquinone - A depigmenting agent that may interrupt melanin synthesis.
3. Tretinoin - A keratolytic agent that helps in skin cell turnover.

The exact mechanism of action in treating melasma is not fully understood.

Dosage & Administration

• Application: Apply once daily at night, at least 30 minutes before bedtime.
• Instructions:
  • Gently wash your face and neck with a mild cleanser, rinse, and pat dry.
  • Apply a thin film of the cream to the hyperpigmented areas, including about ½ inch of normal-appearing skin around each lesion.
  • Rub lightly and uniformly into the skin.
  • During the day, use sunscreen and protective clothing, and avoid sunlight exposure.
• Pediatric Use: Not established for pediatric patients.

Interaction

• Avoid using medicated or abrasive soaps, cleansers, or cosmetics with drying effects.
• Avoid products with high alcohol content, astringents, or other irritants.
• Do not use photosensitizing medications.

Contraindications

Contraindicated in individuals with a history of hypersensitivity, allergy, or intolerance to any of its components.

Pregnancy & Lactation

• Pregnancy: Category C. Use only if the potential benefit justifies the risk to the fetus due to the presence of Tretinoin, which may cause serious fetal harm.
• Lactation: Caution is advised as it is unknown if the active ingredients can be excreted in human milk.

Precautions & Warnings

• Local Irritation: The cream may cause mild to moderate irritation. If hypersensitivity or chemical irritation occurs, discontinue use.
• Systemic Absorption: Can cause reversible HPA axis suppression, Cushing's syndrome, hyperglycemia, and glucosuria due to corticosteroid absorption. Discontinue use if these occur.

Storage:

• Store in a cool, dry place, protected from light.
• Do not freeze.
• Keep out of reach of children.
