✔️ Pharmacology

Pharmacodynamics: Miconazole exhibits antifungal activity against common dermatophytes and yeasts. It also has antibacterial action against certain gram-positive bacilli and cocci. The main mechanism of action involves the inhibition of a demethylation step in ergosterol biosynthesis, a crucial process for fungal growth. Ergosterol is an essential component of the fungal cell membrane, and its disruption leads to the formation of gaps in the cell membrane. This disruption allows essential constituents of the fungal cells to leak out, eventually causing the death of the fungal cells.

Pharmacokinetics: When administered orally, Miconazole has low bioavailability (25-30%) due to limited absorption from the intestinal tract. After administration as an oral gel, Miconazole is systemically absorbed. Once absorbed, the majority of Miconazole is bound to plasma proteins (88.2%), primarily to serum albumin and red blood cells (10.6%). The absorbed portion of Miconazole is extensively metabolized in the body. Only a very small amount (less than 1%) of the administered dose is excreted unchanged in the urine. The terminal plasma half-life of Miconazole is around 20-25 hours in most patients. The elimination half-life of Miconazole remains similar in patients with renal impairment.

✔️ Dosage & Administration of Gelora Oral Gel

The administration of Gelora Oral Gel is specific to the affected area in the mouth or throat. Here are the recommended doses for both adults and children:

Adult Dose:

  • Oropharyngeal candidiasis and Intestinal candidiasis: The usual dose is 15 mg/kg/day in divided doses. For this gel formulation, one or two teaspoons (5-10 ml) of gel should be taken four times daily. It is recommended to continue treatment for at least 1 week after the symptoms have disappeared.
  • For localized lesions of the mouth: A small amount of gel should be applied directly to the affected area with a clean finger four times daily for 5-7 days.

Child Dose:

  • Oropharyngeal candidiasis:
    • Children aged 6 years and over One teaspoonful (5 ml) of gel four times daily.
    • Children aged 2-6 years: One teaspoonful (5 ml) of gel twice daily.
    • Infants under 2 years: Half teaspoonful (2.5 ml) of gel twice daily.
    • Treatment should be continued for at least 1 week after the symptoms have disappeared.
  • For localized lesions of the mouth:
    • Smear a small amount of gel on the affected area with a clean finger four times daily for 5-7 days.
