Description:

Extranil 5mg Tablet contains Procyclidine which belongs to the group of medicines called Anticholinergics. It is used to relieve and manage the signs of Parkinson's disease such as stiff muscles, paralysis, shaking hands (tremors), trouble talking, writing, and walking, producing too much saliva and dribbling, sweating, uncontrolled eye movements, and depression.

Extranil 5mg Tablet is also used to control extrapyramidal effects caused by certain medicines used to manage mental health conditions, which might include restlessness and unusual head and body movements.

Extranil 5mg Tablet is not recommended for use if you have tardive dyskinesia (uncontrolled movements of the head or tongue). 

Extranil 5mg Tablet should be used with caution in patients with liver, kidney disease, high blood pressure, heart disease, glaucoma, and thyrotoxicosis. Consult your doctor before taking it.

How does Extranil 5mg Tablet work?

Procyclidine hydrochloride, an antimuscarinic agent, treats parkinsonism by correcting the relative cholinergic excess resulting from dopamine deficiency. It is quickly absorbed from the GI tract and cleared from tissues, with effects apparent within 5 to 20 minutes post-intravenous administration and lasting up to 4 hours.

Indications:

Extranil 5 mg (Procyclidine) is used as an adjunctive treatment for various forms of Parkinsonian syndrome. This includes idiopathic Parkinson's disease (paralysis agitans), postencephalitic parkinsonism, and arteriosclerotic parkinsonism. It is also effective in managing extrapyramidal symptoms such as pseudoparkinsonism, acute dystonic reactions, and akathisia caused by neuroleptic drugs like phenothiazine derivatives.

Pharmacology

Procyclidine hydrochloride is an antimuscarinic antiparkinsonian agent with relatively low toxicity. It is a synthetic tertiary amine. The drug alleviates Parkinsonian symptoms by addressing the relative excess of cholinergic activity that arises due to dopamine deficiency. It is absorbed from the gastrointestinal tract and is rapidly cleared from the tissues. Intravenous administration results in an effect within 5 to 20 minutes, lasting up to 4 hours.

Dosage & Administration of Extranil 5mg Tablet

Oral Administration:

  • Adults:
    • Parkinsonism: Start with 2.5 mg three times daily, increasing to 5 mg three times daily, with an additional 5 mg at bedtime if needed. Adjust dosage as tolerated, up to a total daily dose of 20 to 30 mg divided into 3 to 4 doses.
    • Drug-induced Extrapyramidal Symptoms: Start with 2.5 mg three times daily. Increase by 2.5 mg daily increments as needed and tolerated.

Children:

  • Safety and efficacy in pediatric patients have not been established. Use only if the potential benefits outweigh the risks.
Procyclidine may interact with:
  • Phenothiazines
